摘要
目的探讨留置骨水泥间隔器应用于老年人工关节感染(PJI)的疗效。方法按照随机数字表法选择河南省洛阳正骨医院2018年1月至2021年1月收治的80例老年PJI患者的临床资料进行回顾性分析,均留置骨水泥间隔器,留置时间6个月以上,调查治疗与随访情况。结果80例患者均获得半年以上随访,随访时间6~18(11.38±2.25)个月,其中生存77例,死亡3例,生存率为96.25%(77/80);77例生存患者中感染控制有效74例,无效3例,感染控制有效率为96.10%;80例患者随访期间发生并发症7例,并发症发生率为8.75%(7/80);80例患者出院时与随访半年时髋关节功能评分、膝关节评分及疼痛视觉模拟评分法评分比较,差异均有统计学意义(P<0.05)。结论老年PJI患者留置骨水泥间隔器处理,可有效控制感染,改善髋关节与膝关节功能,并发症少。
Objective To investigate the efficacy of indwelling bone cement spacer on prosthetic joint infection(PJI)in elderly.Methods Eighty elderly PJI patients admitted to Luoyang Orthopedic Hospital of Henan Province from January 2018 to January 2021 were randomly selected for retrospective analysis.The clinical data of the included subjects were not missing,and they were all indwelled with bone cement spacers for more than 6 months.The treatment and follow-up was investigated.Results Eighty patients were followed up for more than half a year,with an average follow-up of 6~18(11.38±2.25)months.Among them,77 cases survived and 3 cases died,and the survival rate was96.25%(77/80);among the 77 cases surviving patients,74 cases were effective and 3 cases were ineffective.The effective rate of infection control was 96.10%;a total of 7 complications were found in80 patients during the follow-up period,and the complication rate was 8.75%.There were significant differences in the hip joint function score,knee joint score and pain VAS score of 80 patients between discharged and followed up for half a year(P<0.05).Conclusions The treatment of elderly PJI patients with indwelling bone cement spacer can effectively control the infection.It can well improve the function of the hip and knee joints,with fewer complications.
