摘要
糖尿病大血管病变是糖尿病致死致残的主要原因。现代研究发现,由高糖引发的级联反应难以通过单纯的降低血糖而逆转,因而当前以强化降糖为主的糖尿病大血管病变防治效果并不尽如人意,如何防控并尽早使患者的大血管获益,创新有效防治糖尿病大血管病变的诊疗技术,是当今研究热点也是亟待突破的技术瓶颈。我们以传统中医“伏邪”理论为基础,与现代西医“代谢记忆”认识相结合,系统梳理了中医对糖尿病大血管病变发生发展的认识,建立了独特的“伏邪-代谢记忆-糖尿病大血管病变”认识体系,并从证候、治法、组方、药物、疗效、机制等多角度开展了全方位的一体化研究,初步搭建起伏邪理论指导下的糖尿病大血管病变中医药防治体系构架,以开拓临床糖尿病大血管病变的防控思路,并以此为示范,探索传统中医伏邪理论对现代慢性重大疑难疾病干预指导的途径与方法。
Diabetic macrovascular disease is the main cause of death and disability in diabetes mellitus.Modern research has found that the cascade reaction triggered by high glucose is difficult to reverse by simply lowering blood glucose.Therefore,the current prevention and treatment of diabetic macrovascular disease with intensive glucose lowering is not satisfactory.How to improve the patients'macrovascular benefits and innovate the effective diagnosis and treatment technology of diabetic macrovascular disease are hot topics and urgent technical bottlenecks that need to be broken through.Based on the theory of"incubative pathogen"in TCM and combined with the understanding of"metabolic memory"in modern western medicine,we have systematically reviewed the understanding of TCM in the occurrence and development of diabetic macrovascular disease,and established a unique cognition system of"incubative pathogen-metabolic memory-diabetic macrovascular disease".It has also carried out an all-round integrated research from the perspectives of syndrome,treatment,prescription,medicine,curative effect and mechanism.We have initially established a system framework of"incubative pathogen-metabolic memory-diabetic macrovascular disease"to develop the prevention and control of clinical diabetic macrovascular disease;And we take this as a demonstration to explore the ways and methods of TCM theory of incubative pathogen in the intervention and guidance of modern chronic difficult diseases.
