摘要
目的探讨血清类风湿因子(RF)、免疫球蛋白(Ig)、抗核抗体(ANA)与抗可提取性抗原(ENA)抗体检测对类风湿关节炎(RA)疾病活动度的诊断效能。方法选择2021年1—12月于张家港市中医医院就诊的100例RA患者作为研究组,另外选择同期50名健康体检者作为对照组。检测并分析两组血清RF、ANA、抗ENA抗体的阳性率及IgA、IgG、IgM水平差异。将研究组患者根据RA疾病活动度评价表(DAS28)分为RA轻度活动组(32例)、RA中度活动组(44例)、RA重度活动组(24例),比较3个亚组血清RF、ANA、抗ENA抗体的阳性率及IgA、IgG、IgM水平差异。采用Spearman相关分析法进一步评估RA疾病活动度与血清RF、ANA、抗ENA抗体的阳性率及IgA、IgG、IgM水平的相关性。结果研究组血清RF、ANA、抗ENA抗体的阳性率和IgA、IgG、IgM水平均明显高于对照组〔RF:81.00%(81/100)比2.00%(1/50),ANA:19.00%(19/100)比0%(0/50),抗ENA抗体:17.00%(17/100)比0%(0/50),IgA(g/L):3.20±1.01比2.14±0.53,IgG(g/L):15.23±4.08比13.70±4.17,IgM(g/L):1.63±0.42比1.15±0.20,均P<0.05〕。随着RA病情发展,RA重度活动组血清RF、ANA、抗ENA抗体阳性率及IgA、IgG、IgM水平均明显高于RA轻度活动组和RA中度活动组〔RF:29.16%(7/24)比6.25%(2/32)、6.81%(3/44),ANA:20.83%(5/24)比3.12%(1/32)、4.54%(2/44),抗ENA抗体:12.50%(3/24)比0%(0/32)、2.27%(1/44),IgA(g/L):3.76±1.08比2.15±0.77、2.86±0.84,IgG(g/L):16.23±3.08比13.86±3.59、14.27±3.54,IgM(g/L):1.75±0.33比1.23±0.21、1.43±0.24,均P<0.05〕。Spearman相关性分析结果显示,血清RF、ANA、抗ENA抗体阳性率及IgA、IgG、IgM水平均与RA疾病活动度呈正相关(r值分别为0.653、0.556、0.486、0.521、0.510、0.497,P值均为0.001)。结论血清指标联合检测可反映RA患者的病情严重程度,为临床治疗提供数据支持。
Objective To investigate the diagnostic efficiency of serum rheumatoid factor(RF),immunoglobulin(Ig),antinuclear antibody(ANA)and anti-extractable nuclear antigen(ENA)antibody detection for the disease activity of rheumatoid arthritis(RA).Methods A total of 100 patients with RA who were treated in Zhangjiagang Hospital of Traditional Chinese Medicine from January to December 2021 were selected as the research group,and 50 healthy subjects during the same period were selected as the control group.The differences in positive rates of serum RF,ANA,anti-ENA antibody and levels of IgA,IgG,IgM between the two groups were detected and analyzed.The patients in research group were divided into RA mild activity group(32 cases),RA moderate activity group(44 cases)and RA severe activity group(24 cases)according to disease activity score derivative for 28 joints(DAS28).The positive rates of serum RF,ANA,anti-ENA antibody and the levels of IgA,IgG,IgM among the three groups were compared.Spearman correlation analysis was used to further evaluate the correlation between RA disease activity and the positive rates of serum RF,ANA,anti-ENA antibodies and the levels of IgA,IgG and IgM.Results The positive rates of serum RF,ANA,anti-ENA antibody and the levels of IgA,IgG,IgM in research group were higher than those in control group[RF:81.00%(81/100)vs.2.00%(1/50),ANA:19.00%(19/100)vs.0%(0/50),anti-ENA antibody:17.00%(17/100)vs.0%(0/50),IgA(g/L):3.20±1.01 vs.2.14±0.53,IgG(g/L):15.23±4.08 vs.13.70±4.17,IgM(g/L):1.63±0.42 vs.1.15±0.20,all P<0.05].The positive rates of serum RF,ANA,anti-ENA antibody and the levels of IgA,IgG,IgM in RA severe activity group were higher than those in RA mild activity group and RA moderate activity group[RF:29.16%(7/24)vs.6.25%(2/32),6.81%(3/44),ANA:20.83%(5/24)vs.3.12%(1/32),4.54%(2/44),anti-ENA antibody:12.50%(3/24)vs.0%(0/32),2.27%(1/44),IgA(g/L):3.76±1.08 vs.2.15±0.77,2.86±0.84,IgG(g/L):16.23±3.08 vs.13.86±3.59,14.27±3.54,IgM(g/L):1.75±0.33 vs.1.23±0.21,1.43±0.24,all P<0.05].The results
