摘要
目的:分析肩胛上神经阻滞联合盂肱关节类固醇注射治疗粘连性肩关节囊炎患者的效果。方法:选择2018年5月—2020年5月我院收治的92例粘连性肩关节囊炎患者作为研究对象,通过随机数字表法分为常规组和联合组,每组46例。常规组给予常规康复治疗,联合组在常规组基础上加用超声引导肩胛上神经阻滞联合盂肱关节类固醇注射治疗。比较两组临床疗效、肩部疼痛程度和Constant肩关节评分(CSS)、肩关节功能恢复情况及肩部活动情况。结果:联合组治疗有效率为95.65%,高于常规组的73.91%(P<0.05);联合组VAS评分低于常规组,CSS评分高于常规组(P<0.05);联合组UCLA评分高于常规组(P<0.05);联合组胳膊外旋角度、外展角度、上举角度高于常规组(P<0.05)。结论:粘连性肩关节囊炎患者应用肩胛上神经阻滞、盂肱关节类固醇注射联合治疗,疗效显著,能够明显改善患者关节功能,提高关节活动度,缓解疼痛。
Objective:To analyze the effect of suprascapular nerve block combined with glenohumeral joint steroid injection in the treatment of adhesive shoulder bursitis.Methods:A total of 92 patients with adhesive scapulohumeral periarthritis admitted to our hospital from May 2018 to May 2020 were selected as subjects.They were divided into routine group and combined group by random number table method,with 46 cases in each group.The conventional group was treated with routine rehabilitation,and the combined group was treated with ultrasound-guided suprascapular nerve block combined with steroid injection of glenohumeral joint on the basis of the conventional group.The clinical efficacy,shoulder pain,Constant shoulder score(CSS),shoulder function recovery and shoulder activity were compared between the two groups.Results:The effective rate of the combined group was 95.65%,which was higher than 73.91% of the conventional group(P<0.05).The VAS score of the combined group was lower than that of the conventional group,and the CSS score was higher than that of the conventional group(P<0.05).UCLA score in the combined group was higher than that in the conventional group(P<0.05);the arm rotation angle,abduction angle and lifting angle in the combined group were higher than those in the conventional group(P<0.05).Conclusion:The combination of suprascapular nerve block and glenohumeral joint steroid injection in the treatment of adhesive shoulder arthritis can significantly improve the patient’s joint function,improve the range of motion and relieve pain.
