摘要
目的通过CT三维重建对八髎穴针刺方法进行研究。方法对26例女性患者及志愿者分别针刺次髎、中髎穴后进行CT三维重建1次。结果八髎穴定位,应根据L4和L5棘突间顺延触摸,以骶椎棘突为体表标志确定水平线,结合既往旁开距离参数确定八髎穴所在垂直线;针刺操作上,针刺角度自S1~S4按一定规律依次递加,且针刺深度以患者产生向小腹部、会阴区传导的有效针感为准。结论经过临床及CT三维重建再验证的八髎穴定位及针刺方法,基本可以保证针刺准确进入骶后孔。
Objective To observe the method of needle insertion into Baliao points by CT three-dimensional reconstruction.Method In 26 female patients and volunteers,CT three-dimensional reconstruction was completed twice after Ciliao point and Zhongliao point were acupunctured,respectively.Result Baliao points should be located by touching downwards along L4 and L5 spinous processes and determining the horizontal line according to the spinous process of the sacral vertebra as a body surface mark and the perpendicular line according to the previous lateral distance parameter.Acupuncture should be performed by increasing the angle of needle insertion in order from S1 to S4 according to certain regularity.The depth of needle insertion should be subject to the effective needling sensation transmitted to the lower abdomen and perineum in the patient.Conclusion The location of Baliao points and acupuncture method by clinical practice and again by CT three-dimensional reconstruction can basically ensure the accurate insertion of needles into posterior sacral foramina.
