摘要
目的探究超声骨刀对下颌阻生齿拔除患者疗效及疼痛标志物、炎症反应、下牙槽神经损伤的影响。方法选取2021年5月-2022年3月南通大学附属海安医院收治的62例行下颌阻生齿拔除患者,按照治疗方法不同分为对照组(n=31,高速涡轮微创拔除)和研究组(n=31,超声骨刀拔除),比较两组手术时间、出血量、开口度、术后肿胀程度、龈沟液疼痛标志物[降钙素基因相关肽(CGRP)、P物质(SP)、5-羟色胺(5-HT)]、炎症因子水平[肿瘤坏死因子-ɑ(TNF-ɑ)、过氧化物酶(MPO)、细胞间粘附因子1(ICAM1)]及并发症发生情况。结果研究组手术时间、出血量、肿胀程度低于对照组,开口度高于对照组(P<0.05);术后3d两组龈沟液疼痛标志物CGRP、SP、5-HT均升高及炎症介质TNF-ɑ、MPO、ICAM1水平均升高,且研究组低于对照组(P<0.05);研究组术后总并发症发生率低于对照组(P<0.05)。结论超声骨刀对下颌阻生齿拔除患者疗效确切,能缩短手术时间,减少出血量和下牙槽神经损伤,改善疼痛和炎症反应水平。
Objective To investigate the effect of ultrasonic osteotome on pain markers,inflammatory re-sponse,and inferior alveolar nerve injury in patients undergoing extraction of impacted mandibular teeth.Methods Sixty-two patients who underwent extraction of impacted mandibular teeth in our hospital from May 2021 to March 2022 were selected and divided into a control group(n=31,micro invasive extraction of high-speed turbine)and a study group(n=31,extraction by ultrasonic osteotome)according to different treatment methods.The general surgical condi-tions(operation time,blood loss),opening degree,postoperative swelling degree,gingival crevicular fluid pain markers[calcitonin gene-related peptide(CGRP),substance P(SP),serotonin(5-HT)],inflammatory factor levels[tumor necro-sis factor-(TNF-α),peroxidase(MPO),intercellular adhesion factor 1(ICAM1)],and complications were compared be-tween the two groups.Results The operation time and blood loss in the study group were lower than that in the con-trol group(P<0.05).The opening degree in the study group was higher than that in the control group 3 days after opera-tion,but the swelling degree was lower than that in control group(P<0.05).The gingival crevicular fluid pain markers CGRP,SP,5-HT and the levels of inflammatory mediators TNF-α,MPO,ICAM1 were increased in both groups 3 days after operation,and the study group was lower than the control group(P<0.05).The incidence rate of lower lip numb-ness in study group was lower than that in control group(P<0.05).Conclusions ultrasonic osteotome is effective in the extraction of impacted mandibular teeth.It can shorten the operation time,reduce the blood loss and inferior alveolar nerve injury,and improve the intensity of pain and inflammatory reaction.
