摘要
目的双侧开窗减压与全椎板切除内固定治疗老年退变性椎管狭窄的疗效对比。方法回顾性分析仙桃市中医医院2018年1月—2019年12月的100例老年退变性椎管狭窄患者,根据治疗模式的差异分为对照组以及观察组,每组50例,对照组实施全椎板切除内固定治疗,观察组给予双侧开窗减压内固定治疗。比较治疗前后Oswestry评分、视觉模拟评分、JOA评分、椎管直径、生活自理能力、治疗效果。结果治疗后观察组Oswestry评分(11.25±1.25)分、视觉模拟评分(1.24±0.15)分显著低于对照组,JOA评分(21.56±3.46)分、椎管直径(15.21±3.22)mm和生活自理能力(96.52±4.12)分均显著高于对照组,差异有统计学意义(t=7.326、14.830、9.633、6.728、13.910,P<0.05)。观察组中患者治疗效果为100.00%(50/50),高于对照组的74.00%(37/50),差异有统计学意义(χ^(2)=14.943,P<0.05)。结论老年退变性椎管狭窄患者采用双侧开窗减压内固定可更好地改善椎管狭窄的情况,有利于减轻腰椎疼痛和改善患者的腰椎功能,提高其生活自理能力。
Objective To compare the efficacy of bilateral fenestration decompression and total laminectomy and internal fixation in the treatment of senile degenerative spinal stenosis.Methods Retrospective analysis of 100 elderly patients with degenerative spinal stenosis in Xiantao Traditional Chinese Medicine Hospital from January 2018 to December 2019,and they were divided into control group and observation group according to the difference in treatment mode,with 50 cases in each group.The control group was given total laminectomy and internal fixation,and the observation group was given bilateral fenestration decompression and internal fixation.The Oswestry score,visual analog score,JO A score,spinal canal diameter,self-care ability and treatment effect were compared before and after treatment.Results After treatment,the Oswestry score(11.25±1.25)points and visual analog score(1.24±0.15)points of the observation group were significantly lower than those of the control group,the JO A score(21.56±3.46)points,spinal canal diameter(15.21±3.22)mm and self-care ability(96.52±4.12)points in the observation group were significantly higher than those in the control group,the difference was statistically significant(t=7.326,14.830,9.633,6.728,13.910,P<0.05).The therapeutic effect of patients in the observation group was 100.00%(50/50)higher than that in the control group74.00%(37/50),and the difference was statistically significant(χ^(2)=14.943,P<0.05).Conclusion Bilateral fenestration decompression and internal fixation in elderly patients with degenerative spinal canal stenosis can better improve the situation of spinal stenosis,help to relieve lumbar pain,improve the lumbar function of patients,and improve their self-care ability.
