摘要
目的观察应用肌内效贴联合肩胛带控制训练治疗脑卒中后偏瘫侧肩关节半脱位的效果.方法将52例脑卒中后并发偏瘫侧肩关节半脱位的患者随机分成对照组(n=26)和观察组(n=26).两组患者均给予常规康复治疗,观察组患者增加肌内效贴联合肩胛带控制训练方案.分别于治疗前和治疗6周后采用X线测量法测量患侧肩峰至肱骨头间距(AHI)、疼痛视觉模拟评分(VAS)法评定肩痛、简化Fugl-Meyer评分量表上肢部分(FMA)评定上肢功能.结果治疗前两组患者AHI值、VAS评分和FMA评分组间差异无统计学意义(P>0.05).治疗6周后两组患者AHI数值和VAS评分比治疗前降低(P<0.05),且观察组比对照组降低更明显,差异有统计学意义(P<0.05);两组患者FMA评分比治疗前增高(P<0.05),且观察组增高比对照组更明显,差异有统计学意义(P<0.05).结论在常规康复治疗基础上采用肌内效贴联合肩胛带控制训练能够有效减轻脑卒中后偏瘫侧肩关节半脱位程度,缓解患侧肩痛并促进上肢运动功能的恢复.
Objective To observe the curative effect of Kinesio taping method combined with scapular control training in the treatment of subluxation of shoulder in hemiplegic side after stroke.Methods 52 stroke patients with shoulder subluxation were randomly divided into the control group(n=26)and the treatment group(n=26).Both groups were administered with routine rehabilitation treatment,and the patients in the treatment group accepted Kinesio taping method combined with scapular control training program.All the patients were assessed using X-rays for Acromio-humeral interval(AHI),Visual analogue scale(VAS)for shoulder pain,simplified Fugl-Meyer Assessment(FMA)for upper extremity function before and 6 weeks of treatment respectively.Evaluate and analyze the therapeutic effect of the two groups.Results There was no significant difference in AHI,VAS and FMA between the two groups before treatment(P>0.05).After 6 weeks of treatment,AHI and VAS scores of the two groups were significantly lower than before(P<0.05),and the treatment group was significantly lower,with statistical significance(P<0.05).The scores of FMA in both groups were higher than those before the treatment(P<0.05),and those in treatment group were higher than those in control group,with statistical significance(P<0.05).Conclusion On the basis of routine rehabilitation treatment,Kinesio taping method combined with scapular control training can effectively treat the shoulder subluxation,alleviate shoulder pain of affected side and promote the recovery of upper limb motor function.
