高频超声诊断急性踝内翻腓骨肌腱脱位的临床价值

摘要 目的探讨高频超声诊断急性踝内翻损伤腓骨肌腱脱位的临床价值。方法回顾性分析临床诊断为急性踝关节内翻损伤腓骨肌腱脱位患者18例,且行踝关节高频超声检查及MRI检查观察腓骨肌腱脱位情况,比较超声与MRI诊断的一致性。结果18例腓骨肌腱脱位患者超声均作出准确诊断,其中I型脱位10例,II型脱位4例,III型脱位3例,IV型脱位1例;MRI诊断腓骨肌腱脱位12例,其中I型脱位5例,II型脱位4例,III型脱位3例。结论急性内踝关节扭伤导致腓骨肌腱脱位,高频超声相对于其他影像学检查方法,具有分辨率高、实时动态、操作简便的优点,应作为临床诊断腓骨肌腱脱位的首选检查,值得临床推广应用。 Objective To explore the clinical value of high-frequency ultrasound in the diagnosis of peroneal tendon dislocation when acute ankle inversion sprain occurred.Methods A retrospective analysis was performed on 18 patients with acute ankle inversion sprain with peroneal tendon dislocation,and high-frequency ultrasound imaging and MRI imaging were performed to observe the peroneal tendon dislocation,and the consistency of ultrasound and MRI diagnosis was compared.Results All 18 patients with peroneal tendon dislocation were diagnosed accurately by ultrasound,including 10 type I dislocations,4 type II dislocations,3 type III dislocations,and 1 type IV dislocation respectively,and 12 cases with peroneal tendon dislocation were diagnosed by MRI accurately,including 5 type I dislocations,4 type II dislocations,3 type III dislocations respectively.Conclusion Compared with other imaging methods,high frequency ultrasound has the advantages of high resolution,real-time dynamic and easy operation,and should be used as the first choice for clinical diagnosis of peroneal tendon dislocation caused by acute internal ankle sprain,so it is worthy of clinical application.
