摘要
目的深入挖掘国医大师张大宁教授治疗CKD3期患者的用药配伍规律,为CKD3期患者的中医药特色治疗提供新的思路和参考。方法通过回顾性研究的方法,收集张大宁教授诊治的108例CKD3期患者的完整医案并从中提取中药处方,运用规则分析、改进互信息法等数据挖掘方法分析其用药规律。结果研究纳入的305首中药处方共涉及101味中药。使用频次较多的中药为生黄芪、土茯苓、川芎、丹参、五灵脂、五味子等;药性以温、寒为主;药味以甘、苦为主;归经以肝经、脾经、胃经、肾经为多;核心处方为:生黄芪,土茯苓,丹参,川芎,五味子,茵陈,五灵脂,蒲黄炭;得到药物核心组合6对、新处方6首。结论本研究从多角度科学、量化的分析了张大宁教授治疗慢性肾脏病的“补肾活血、化湿降浊”的理论内涵,为CKD3期患者的临床治疗提供中医方案。
Objective To deeply explore the drug compatibility rules of TCM master Professor Zhang Daning in treating patients with stage 3 CKD, and to provide new ideas and references for TCM characteristic treatment of patients with stage 3 CKD.Methods A retrospective study was conducted to collect the complete medical records of 108 CKD stage 3patients diagnosed and treated by Professor Zhang Daning and extract TCM prescriptions. Rule analysis, improved mutual information method and other data mining methods were used to analyze the rule of medication.Results A total of 305 TCM prescriptions were included in this study, involving 101 TCM flavors. Among them, the most frequently used herbs were Raw Radix Astragali seu Hedysari, Rhizoma Smilacis Glabrae, Rhizoma Ligustici Chuanxiong, Radix Salviae Miltiorrhizae, Faeces Togopteri, Fructus Schisandrae Chinensis, etc. The main medicinal properties were warm and cold.The taste of medicine was mainly sweet and bitter. The main meridians were liver, spleen, stomach and kidney. The core prescriptions were: Raw Radix Astragali seu Hedysari, Rhizoma Smilacis Glabrae, Radix Salviae Miltiorrhizae, Rhizoma Ligustici Chuanxiong, Fructus Schisandrae Chinensis, Herba Artemisiae Scopariae, Faeces Togopteri, charcoal Pollen Typhae;6 pairs of drug core combinations and 6 new prescriptions were obtained.Conclusion This study scientifically and quantitatively analyzes the theoretical connotation of Professor Zhang Daning′s "tonifying kidney and promoting blood circulation, resolving dampness and reducing turbidity" in the treatment of chronic kidney disease from multiple perspectives, so as to provide a traditional Chinese medicine scheme for the clinical treatment of CKD3 patients.
