摘要
目的分析老年高血压患者颈动脉内膜中层厚度(IMT)与甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、尿酸(UA)及同型半胱氨酸(HCY)等代谢指标的相关性,为患者防治动脉粥样硬化(AS)提供临床依据。方法根据颈动脉IMT,120例原发性老年高血压患者分为A组(IMT正常)、B组(IMT增厚)和C组(斑块),选择同期体检的健康人群120例作为对照组。比较各组间TC、TG、LDL-C、HDL-C、UA、HCY水平,并分析上述指标与患者颈动脉IMT的相关性。结果高血压患者的SBP、DBP、TC、TG、LDL-C、UA、HCY水平高于对照组,HDL-C水平低于对照组,差异均有统计学意义(均P<0.05)。A组、B组、C组高血压患者间TC、TG、LDL-C、HDL-C、UA、HCY水平,差异有统计学意义(均P<0.05),均为A组优于B组、B组优于C组。Pearson相关分析结果显示,高血压患者TC、TG、LDL-C、UA、HCY水平与颈动脉IMT呈正相关(均P<0.05),HDL-C与颈动脉IMT呈负相关(r=-0.79,P<0.05)。结论高血压患者颈动脉IMT与TC、TG、LDL-C、HDL-C、UA、HCY水平均有明显相关性。
Objective To analyze the correlation between carotid intima-media thickness(IMT)and metabolic indexes such as triglyceride(TG),total cholesterol(TC),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C),uric acid(UA),and homocysteine(HCY)in elderly patients with hypertension to provide the clinical basis for the prevention and treatment of atherosclerosis(AS).Methods According to carotid IMT,120 elderly patients with essential hypertension were divided into group A(average IMT),group B(IMT thickening),and group C(plaque).120 healthy people who underwent a physical examination at the same time were selected as the control group.The levels of TC,TG,LDL-C,HDL-C,UA,and HCY were compared among the groups,and the correlation between these indexes and carotid IMT was analyzed.Results The levels of SBP,DBP,TC,TG,LDL-C,UA,and HCY in patients with hypertension were higher than those in the control group,and the levels of HDL-C were lower than those in the control group.The differences were statistically significant(all P<0.05).There were significant differences in TC,TG,LDL-C,HDL-C,UA,and HCY levels among patients with hypertension in groups A,B,and C(all P<0.05).Group A was better than group B,and group B was better than group C.Pearson correlation analysis showed that the levels of TC,TG,LDL-C,UA,and HCY in patients with hypertension were positively correlated with carotid IMT(all P<0.05),and HDL-C was negatively correlated with carotid IMT(r=-0.79,P<0.05).Conclusions Carotid IMT was significantly correlated with TC,TG,LDL-C,HDL-C,UA,and HCY levels in patients with hypertension.
