摘要
目的探讨非特殊型浸润性乳腺癌(IBC-NST)临床病理特征以及同侧腋窝淋巴结转移与Ki-67表达的关系。方法回顾性收集经病理证实的IBC-NST患者101例,分为Ki-67高表达(70例)和Ki-67低表达(31例)两组。患者临床病理特征等计数资料的组间比较采用χ~2检验方法及Kruskal-Wallis H检验方法,超声诊断同侧腋窝淋巴结转移与病理结果的比较采用灵敏度、特异度等相关值计算,同侧腋窝淋巴结转移与Ki-67表达的相关性研究采用Spearman相关分析。结果不同的Ki-67表达分组中,肿块直径大小、乳腺癌组织学分级及临床分期情况在Ki-67表达组之间差异具有统计学意义(P<0.05),肿块直经≥2 cm(58.57%)、组织学分级Ⅲ级(32.86%)、临床分期Ⅲ期(34.29%)及Ⅳ期(5.71%)的表达阳性率在Ki-67高表达组中较高;同侧腋窝淋巴结转移与Ki-67高表达呈正相关(r=0.393,P<0.05)。结论IBC-NST病例中,肿块直经≥2 cm、组织学分级Ⅲ级、临床分期Ⅲ期及Ⅳ期与Ki-67高表达有相关性,患者同侧腋窝淋巴结转移与Ki-67高表达呈正相关,为IBC-NST增殖评估和临床干预提供参考。
Objective To investigate the clinicopathological characteristics of non-specific invasive breast cancer(IBC-NST)and the relationship between ipsilateral axillary lymph node metastasis and Ki-67 expression.Methods A total of 101 patients with IBC-NST were retrospectivily recruited and divided into two groups with high expression of Ki-67(70 cases)and low expression of Ki-67(31 cases).Theχ~2 test and Kruskal-Wallis H test were used to compare the clinical and pathological characteristics and other count data of patients between the groups.The comparison of ultrasound diagnosis of ipsilateral axillary lymph node metastasis and pathological results was calculated using correlation values such as sensitivity and specificity.The correlation between ipsilateral axillary lymph node metastasis and Ki-67 expression was analyzed with Spearman correlation analysis.Results In different Ki-67 expression groups,the size of tumor mass,histological grade of breast cancer,and clinical stage were statistically different between Ki-67 expression groups(P<0.05).The positive expression rate of tumor mass≥2 cm(58.57%),histological gradeⅢ(32.86%),clinical stagesⅢ(34.29%)andⅣ(5.71%)was higher in the Ki-67 high expression group;ipsilateral axillary lymph node metastasis and Ki-67 high expression were positively correlated(r=0.393,P<0.05).Conclusion In IBC-NST cases,the tumor mass≥2 cm,histological gradeⅢ,clinical stageⅢ,andⅣare correlated with the high expression of Ki-67.At the same time,ipsilateral axillary lymph node metastasis and Ki-67 high expression are positively correlated,which provides reference for IBC-NST proliferation assessment and clinical intervention.
