摘要
目的:评估脑卒中后偏瘫患者应用通脉活络祛痰汤联合头针、体针疗法的临床效果。方法:选取夹江县中医医院2018年6月-2020年5月收治的62例脑卒中后偏瘫患者为研究对象,按照随机数字表法将其分为A组与B组,各31例。B组采用头针、体针与西医康复疗法,A组在其基础上联合通脉活络祛痰汤。比较两组治疗前及治疗3个月后中医症候积分(半身不遂、肢体麻木或手足拘急、头晕目眩)、神经功能障碍[神经功能缺损评分(NDS)]、痉挛程度[临床痉挛指数(CSI)]变化、肢体运动功能[Fugl-Meyer运动功能评分(FMA)]、平衡功能(Lindmark平衡反应测试)。结果:A组总有效率高于B组,差异有统计学意义(P<0.05);两组患者治疗3个月后中医症候积分、NDS、CSI均比治疗前明显下降(P<0.05),FMA及Lindmark平衡反应评分比治疗前明显上升(P<0.05);A组治疗3个月后中医症候积分、NDS、CSI明显低于B组(P<0.05),FMA及Lindmark平衡反应评分明显高于B组(P<0.05)。结论:在脑卒中后偏瘫患者中采用通脉活络祛痰汤联合头针、体针治疗效果较佳,可改善神经功能性障碍,减少肌肉痉挛,提高运动功能,利于疾病预后转归。
Objective:To evaluate the clinical effect of Tongmai Huoluo Qutan Decoction combined with scalp acupuncture and body acupuncture on patients with post-stroke hemiplegia.Method:A total of 62 patients with post-stroke hemiplegia treated in Jiajiang County Hospital of Traditional Chinese Medicine from June 2018 to May 2020 were selected as the research subjects,and they were divided into group A and group B with 31 cases in each group according to the random number table method.Group B was treated with scalp acupuncture,body acupuncture and western medicine rehabilitation therapy,and group A was combined with Tongmai Huoluo Qutan Decoction on this basis.The scores of traditional Chinese medicine syndromes (hemiplegia,limb numbness or limb contracture,dizziness),neurological dysfunction[neurologicaldeficit score (NDS)],spasticity[clinical spasticity index (CSI)],limb motor function[Fugl-Meyer motor function score (FMA)]and balance function (Lindmark balance response test) were compared between the two groups before treatment and after 3 months of treatment.Result:The total effective rate in group A was higher than that in group B,and the difference was statistically significant (P<0.05).After 3 months of treatment,the scores of traditional Chinese medicine syndromes,NDS and CSI in the two groups were significantly lower than those before treatment (P<0.05),while the scores of FMA and Lindmark balance response were significantly higher than those before treatment (P<0.05).The scores of traditional Chinese medicine syndromes,NDS and CSI in group A after 3 months of treatment were significantly lower than those in group B (P<0.05),while the scores of FMA and Lindmark balance response were significantly higher than those in group B (P<0.05).Conclusion:Tongmai Huoluo Qutan Decoction combined with scalp acupuncture and body acupuncture has better effects in treating patients with post-stroke hemiplegia,and it can improve the neurological dysfunction,reduce the muscle spasm,enhance the motor function,and benefit the prognos
