A analysis of the correlation between CYP1A2 gene polymorphism and postoperative urinary tract infection in children with hypospadias

摘要 目的 分析CYP1A2基因多态性与尿道下裂患儿术后尿路感染的相关性。方法 回顾性分析2018年1月-2020年12月遵义市第一人民医院184例行一期无管化尿道板纵行切开卷管尿道成形术(TIP)治疗的尿道下裂患儿临床资料,根据术后尿路感染发生情况分为尿路感染组(n=26)及无尿路感染组(n=158),并纳入184名同期入院体检男童作为对照组(经年龄成组匹配)。比较三组基线资料及CYP1A2基因多态性分布情况,利用Logistic回归分析评估影响尿道下裂患儿TIP术后发生尿路感染的影响因素。结果 尿路感染组年龄高于无尿路感染组及对照组(P<0.05);尿路感染组术前阴茎弯曲度≥20°、尿道缺损长度≥2 cm者较无尿路感染组占比高(P<0.05)。三组CYP1A2基因位点rs2069514基因型分布有统计学差异(P<0.05),其中尿路感染组AA基因频率及A等位基因频率高于无尿路感染组及对照组(P<0.05);尿道下裂患儿中,AA基因型发生术后尿路感染相对风险度OR=7.60,携带等位基因A患儿OR=1.88。多因素回归分析显示,尿道缺损长度≥2 cm、CYP1A2 rs2069514基因型为AA均为影响尿道下裂患儿TIP术后发生尿路感染的影响因素(P<0.05)。结论尿道缺损长度及CYP1A2基因位点rs2069514多态性与尿道下裂术后尿路感染发生风险有关,可为临床诊疗提供新方向。 OBJECTIVE To analyze the correlation between CYP1 A2 gene polymorphism and postoperative urinary tract infection in children with hypospadias. METHODS A retrospective analysis was performed on the clinical data of 184 children with hypospadias treated by one-stage nonstented tubularized incised plate urethroplasty(TIP) in the First People′s Hospital of Zunyi between Jan 2018 and Dec 2020. According to wheather postoperative urinary tract infection occurred, the children were divided into the urinary tract infection group(n=26) and non-urinary tract infection group(n=158). And 184 boys under physical examination during the same time period were included as the control group(matched by age). The baseline data and distribution of CYP1 A2 gene polymorphism were compared among the three groups. Multivariate Logistic regression analysis was used to evaluate the independent risk factors for occurrence of urinary tract infection in children with hypospadias after TIP. RESULTS The age of the urinary tract infection group was significantly older than that of the non-urinary tract infection group and control group(P<0.05). The number of children with preoperative penile curvature≥20° and the length of urethral defect≥2 cm in the urinary tract infection group was significantly larger than those of the non-urinary tract infection group(P<0.05). There were significant differences in the genotype distribution at CYP1 A2 locus rs2069514 among the three groups(P<0.05), and the AA genotype frequency and A allele frequency in the urinary tract infection group were significantly higher than those in the non-urinary tract infection group and control group(P<0.05). Among children with hypospadias, the relative risk OR of postoperative urinary tract infection in children carrying AA genotype was 7.60, and that in the children carrying A allele was 1.88.
