摘要
目的探讨新生儿败血症外周血核因子-κB(NF-κB)、中性粒细胞CD11b、簇分化抗原64(CD64)表达及与病情严重程度的关系。方法选取2017年10月-2020年9月于徐州医科大学附属连云港医院新生儿科收治的100例败血症患儿为新生儿败血症组,依据病情严重程度,分为重症组32例,轻症组68例。另选取同期医院分娩的50名健康新生儿为正常对照组。测定两组研究对象外周血NF-κB、CD11b、CD64水平,采用受试者工作特性曲线(ROC)分析不同NF-κB、CD11b、CD64水平对败血症患儿病情程度的评估价值。结果新生儿败血症组患儿外周血NF-κB、CD11b、CD64水平均高于正常对照组(P<0.05);重症组患儿外周血NF-κB、CD11b、CD64水平均高于轻症组(P<0.05)。ROC曲线分析结果显示,NF-κB、CD11b、CD64水平单独诊断重症新生儿败血症的ROC曲线下面积均低于三项指标联合诊断重症新生儿败血症的ROC曲线下面积;三项指标单独诊断时的截断值分别为:17.65 mg/L、386.27平均荧光强度(Median fluorescence intensity,MFI)和45.92 MFI。结论NF-κB、CD11b、CD64在重症败血症患儿外周血中均呈较高水平,三项指标联合检测对重症新生儿败血症具有较好的预测价值。
OBJECTIVE To explore the relationship between the expression of peripheral blood nuclear factor-κB(NF-κB), neutrophil CD11 b and cluster differentiation antigen 64(CD64) in neonates with sepsis and the severity of disease. METHODS A total of 100 neonates with sepsis who were treated in neonatology department of Lianyungang Hospital Affiliated to Xuzhou Medical University from Oct 2017 to Sep 2020 were enrolled in the study and divided into the severe group with 32 cases and the mild group with 68 cases according to the severity of disease. Meanwhile, 50 healthy neonates were chosen as the normal control group. The levels of peripheral blood NF-κB, CD11 b and CD64 of the two groups of study subjects were detected, and the values of the NF-κB, CD11 b and CD64 in assessment of illness condition were analyzed by using receiver operating characteristic(ROC) curve. RESULTS The levels of peripheral blood NF-κB, CD11 b and CD64 of the neonates with sepsis were significantly higher than those of the neonates in the normal control group(P<0.05). The levels of peripheral blood NF-κB, CD11 b and CD64 of the severe group were significantly higher than those of the mild group(P<0.05). ROC curve analysis showed that the area under ROC curve of the single NF-κB, CD11 b or CD64 was less than that of the joint detection of the three indexes in diagnosis of severe sepsis of the neonates;the cutoff values of the three indexes were respectively 17.65 mg/L, 386.27 median fluorescence intensity(MFI) and 45.92 MFI in single diagnosis. CONCLUSION The neonates with severe sepsis show high levels of NF-κB, CD11 b and CD64 in peripheral blood;the joint detection of the three indexes has high value in prediction of severe sepsis.
