摘要
目的探究鼻部韧带保留和重建对隆鼻术后鼻尖形态的影响。方法自2020—2021年,重庆当代整形美容外科医院整形美容外科对58例患者采用外入路鼻整形术,术中行Pitanguy韧带保留及重建,初次手术者离断但保留Pitanguy韧带,完成鼻部支架结构搭建后复位缝合韧带;二次修复者采用自体肌筋膜重建已受损的Pitanguy韧带代替完成其功能。观察术后鼻部愈合情况及近期和远期鼻部外观形态。结果所有患者术后获随访12个月,鼻尖外形圆润挺翘、皮肤活动度较好,鼻尖鼻背衔接自然,鼻尖下旋不明显。结论Pitanguy韧带保留和重建符合鼻部正常解剖结构,对鼻尖鼻背形态的塑造和维持具有可靠的效果,值得临床推广应用。
Objective To explore the effect of combining preservation and reconstruction of nasal ligaments on the shape of nasal tip after rhinoplasty.Methods Our retrospective study between 2020 and 2021 was based on 58 patients who underwent preserving Pitanguy's ligament in the nasal reconstruction through an external rhinoplasty approach.Starting from patients with their original noses,the Pitanguy's ligament was retained after dissection;the nasal ligaments were then repositioned and sutured after the reconstruction of the nasal stent was completed.In patients with reconstructed noses,autologous myofascia was used to restore the damaged Pitanguy's ligament to fulfill their function.The shape of nose in short-term and long-term was observed after operation.The postoperative efficacy was subjectively evaluated by patients.Results All patients were followed up for 12 months after operation.The nasal tip was round and upturned,the skin mobility was good,the nasal tip and nasal dorsum were connected naturally,and the nasal tip downward rotation was not visible.Conclusion The combining preservation and reconstruction of Pitanguy's ligament conforms to the normal anatomical structure of nose,and has reliable efficacy in the reconstruction and maintenance of the shape of both nasal tip and nasal dorsum.It is considerable for further clinical promotion and application.
