血清降钙素原与C反应蛋白在新生儿早发败血症中的诊断价值

摘要 目的 探讨早期C反应蛋白(CRP)与血清降钙素原(PCT)检测在新生儿早发败血症诊治中的应用价值。方法 选取2018年7月至2020年6月本院NICU收治的54例新生儿早发败血症患儿作为观察组,另选取同期本院收治的61例非感染患儿作为对照组。比较两组入院时PCT和CRP水平,比较观察组入院时和恢复期血清PCT、CRP水平。结果 入院时,观察组血清PCT及CRP水平明显高于对照组,差异有统计学意义(P<0.05);观察组恢复期的血清PCT、CRP均低于入院时,差异有统计学意义(P<0.05)。PCT诊断新生儿败血症的灵敏度、特异度、阳性预测值、阴性预测值、AUC分别为66.67%、91.8%、87.8%、75.68%、0.829。CRP诊断新生儿败血症的灵敏度、特异度、阳性预测值、阴性预测值、AUC分别为33.33%、86.89%、69.23%、59.55%、0.648。结论 与C反应蛋白相比,血清PCT对诊断新生儿早发败血症敏感性更高。 Objective To explore the application value of early C-reactive protein(CRP) and serum procalcitonin(PCT) in the diagnosis and treatment of neonatal early sepsis. Methods A total of 54 children with early neonatal septicemia admitted to NICU of our hospital were selected as the observation group, and 61 non-infected children admitted to our hospital in the same period were selected as the control group from July 2018 to June2020. According to preset periods, the blood samples were collected for PCT and CRP determinations, and the serum PCT and CRP levels at admission and convalescence in the observation group were analyzed. Results On admission, the serum PCT and CRP levels in the observation group were significantly higher than those in the control group, and the difference was statistically significant(P<0.05);the serum PCT and CRP of the observation group in the recovery period were lower than those in the admission, and the difference was statistically significant(P<0.05). The sensitivities, specificities, positive predictive values, negative predictive values and AUC of PCT in diagnosing early neonatal septicemia were 66.67%, 91.8%, 87.8%,75.68% and 0.829, respectively. The sensitivities, specificities, positive predictive values, negative predictive values and AUC of CRP in diagnosing early neonatal septicemia were 33.33%, 86.89%, 69.23%, 59.55% and 0.648, respectively. Conclusion Compared with C-reactive protein, serum PCT has more sensitive index to diagnose neonatal early sepsis.
