摘要
目的分析建立稀有血型Rh阴性冰冻血库的临床意义。方法选取2018年6月至2020年6月阿勒泰地区血站的3627名无偿献血者作为研究对象,采用微板法对献血者进行初步筛选并应用间接抗人球蛋白试验确定Rh阴性血液,对已确认完毕的Rh阴性血清的新鲜血液采用冰冻深低温保存技术制备稀有血型Rh阴性冰冻红细胞,分析阿勒泰地区Rh阴性血型鉴定比例、Rh阴性不同血型分布情况,比较阿勒泰地区2018至2020年Rh阴性不同血型各项结果。结果3627例无偿献血人员中,Rh阴性者149名,占比4.11%,Rh阳性者3478名,占比95.89%;149名Rh阴性血液的无偿献血者中,A型占比27.52%,B型占比28.86%,AB型占比18.12%,O型占比25.50%,A、B、O型占比均高于AB型(P<0.05);2018至2020年,A型、O型、B型Rh阴性血液的冷冻制备量、临床应用量及临床应用有效率占比均高于AB型,差异有统计学意义(P<0.05)。结论建立稀有血型Rh阴性冰冻血库能提高该血型的应用有效率,提高同血型的输血成功率及需进行紧急输血治疗患者的治疗成功率,为Rh阴性稀有血型患者提供输血保障,具有重要的临床意义。
Objective To analyze the clinical significance of establishing Rh negative frozen blood bank in rare blood type.Methods From June 2018 to June 2020,3627 unpaid blood donors in blood stations in Altay were selected as the research subjects.Blood donors were screened by micro-plate method and Rh negative blood was determined by indirect anti-human globulin test.Rare blood type Rh negative frozen red blood cells were prepared by cryopreservation of fresh blood with confirmed Rh negative serum.To analyze the proportion of Rh negative blood group identification in Altay,Rh negative distribution of different blood types,compare the results of different Rh negative blood types in Altay from 2018 to 2020.Results Of the 3627 unpaid blood donors 149 cases were Rh negative,accounting for 4.11%,3478 cases of Rh positive,accounting for 95.89%;among 149 unpaid blood donors with Rh negative blood,type A accounts for 27.52%,type B accounts for 28.86%,type AB accounts for 18.12%and type O accounts for 25.50%.Type A,B and O accounted for more than AB(P<0.05).From 2018 to 2020,the frozen preparation volume,clinical ap-plication volume and clinical application efficiency of Rh negative blood of type A,O,and B were higher than those of type AB,and the difference was statistically significant(P<0.05).Conclusion The establishment of Rh negative frozen blood bank in rare blood type can improve the applica-tion efficiency of this blood type,improve the success rate of blood transfusion of the same blood type and the treatment success rate of patients who need emergency blood transfusion,provide blood transfusion guarantee for Rh negative rare blood type patients,it has important clinical significance.
