摘要
目的通过问卷调查形式了解糖尿病患者使用胰岛素注射笔针头现状。方法采用自行设计的问卷对解放军总医院第六医学中心内科病房使用胰岛素注射笔针头治疗的糖尿病患者进行调查。结果221例患者中76.47%的使用者未按要求做到针头“一针一换”;平均注射6.47次后更换针头;46.15%的患者感觉注射增加疼痛感;10.27%的患者注射部位出现硬结,6.79%的患者注射部位出现红肿;39.37%的患者发现未按要求更换针头注射时针头出现弯曲,10.41%出现倒钩;69.23%认为更换针头费时费力、非常麻烦,47.06%认为完全没有必要“一针一换”,44.34%患者认为更换针头带来经济压力,42.53%认为未按要求更换针头与未接受宣教相关。结论糖尿病患者胰岛素注射笔针头重复使用现状仍较严峻,针头重复使用增加患者注射疼痛并带来皮肤损害,影响药物吸收,主要与未进行全面深入的健康宣教及针头费用高昂相关,患者对重复使用针头的危害缺乏深刻理解,导致重视程度和依从性较低,影响治疗效果和舒适度。临床中应加强多形式个体化的宣教,促进患者知信行统一,增强依从性,提高血糖达标率和患者生活质量。
Objective To know the current status of the use of insulin pen needle in diabetic patients and related influenc⁃ing factors by questionnaire survey.Methods An investigation was performed on the use of insulin pen needle in diabetic patients who were hospitalized in the internal medicine ward of the Sixth Medical Center by using the self⁃designed questionnaire.Results Of the 221 patients,76.47%of them failed to change the needle after injection as required.On the average,the needle was changed after 6.47 injections.46.15%of them felt that pain increased after injection.10.27%of them had induration at the injection site,and 6.79%had redness and swelling at the injection site.39.37%of them failed to change the needle when it was bent,and 10.41%had needle barbs.69.23%of them held that it took time and effort and troublesome to replace needles.47.06%of them held that it was completely unnecessary to“change needle”after one injection.44.34%of them thought that needle replacement would bring economic pressure,and 42.53%of them had the opinion that failure to replace needles as required was related to the failure of receiving propaganda and education.Conclusion Repeated use of insulin pen needles in diabetic patients is still serrious,repeated use of needles would only increase the pain and skin damage and affect absorption of drugs.It is mainly related to insuffi⁃cient health education and increased cost of needle replacement.Lacking of full understanding of the hazards of repeated use of nee⁃dles results in low compliance,and affects treatment effect and sense of comfort.Various forms of individualized propaganda and edu⁃cation should be strengthened,so that patient compliance could be enhance,and blood glucose standard rate and patients'quality of life would be improved.
