摘要
目的 探讨中医辨证延续性护理在中老年糖尿病周围神经病变中的应用价值。方法 对2020年3月至2021年6月广州市红十字会医院收治的96例中老年糖尿病周围神经病变患者资料开展回顾性分析,根据不同护理方法分为对照组(46例)和观察组(50例)。对照组采用西医综合护理,观察组在西医综合护理的基础上采用中医辨证延续性护理。比较2组的不良事件发生情况、血糖水平、足部神经功能和周围神经传导速度。结果 与干预前比较,2组在干预3个月后的空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(Hb Alc)、症状评分、感觉评分、反射评分均降低,运动正中、尺神经,以及感觉正中、尺神经的传导速度均加快(P均<0.05);观察组在干预3个月后的FPG、2hPG、Hb Alc、症状评分、感觉评分、反射评分均较对照组降低,上述神经传导速度均较对照组加快(P均<0.05)。观察组的不良事件总发生率低于对照组(P<0.05)。结论 中医辨证延续性护理可有效改善中老年糖尿病周围神经病变患者的血糖水平,促进其足部神经功能恢复,加快周围神经传导速度,且减少不良事件的发生。
Objective To explore the application value of continuous nursing of traditional Chinese medicine(TCM)syndrome differentiation in middle-aged and elderly patients with diabetic peripheral neuropathy.Methods The data of 96 middle-aged and elderly patients with diabetic peripheral neuropathy admitted to Guangzhou Red Cross Hospital from March 2020 to June 2021 were retrospectively analyzed.They were assigned into either a control group(46 cases)or an observation group(50 cases)according to different nursing methods.The control group was treated with comprehensive nursing of western medicine,and the observation group was treated with continuous nursing of TCM syndrome differentiation on the basis of comprehensive western medicine nursing.The incidence of adverse events,blood glucose levels,foot neurologic function,and peripheral nerve conduction velocity were compared between the two groups.Results Compared with before intervention,the fasting plasma glucose(FPG),2 hr postprandial plasma glucose(2 hPG),glycosylated hemoglobin(HbAlc),symptom score,sensory score,and reflex score in both groups after 3 months of intervention were decreased,and the conduction velocities of motor and sensory branches of median and ulnar nerves were increased(all P<0.05).After 3 months of intervention,the FPG,2 hPG,HbAlc,symptom score,sensory score,and reflex score of the observation group were lower than those of the control group,and the above-mentioned nerve conduction velocities were faster than those of the control group(all P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group(P<0.05).Conclusion The application of continuous nursing of TCM syndrome differentiation in middle-aged and elderly patients with diabetic peripheral neuropathy can effectively improve their blood glucose levels,promote the recovery of foot neurologic function,accelerate the peripheral nerve conduction velocity,and reduce the incidence of adverse events.
