摘要
目的:观察健脾外敷法联合仙灵骨葆胶囊在早期酒精性股骨头坏死中的临床疗效和安全性。方法:选取成都体育学院附属体育医院诊治的早期酒精性股骨头坏死患者80例,随机分为外敷组、对照组各40列。对照组患者口服仙灵骨葆胶囊、下肢牵引、髋关节功能锻炼,外敷组患者在对照组治疗基础上联合健脾外敷方外敷治疗,分别于治疗前以及治疗后3个月、6个月、12个月,比较两组患者髋关节Harris评分、视觉模拟评分法(VAS)及影像学评分情况,并在治疗结束后比较两组临床疗效,记录不良反应情况。结果:治疗期间两组患者在Harris评分上随时间均呈上升趋势,存在时间-分组交互效应(P<0.05),其中外敷组上升趋势更加明显,且在治疗前以及治疗后3个月两组患者组间差异无统计学意义(P>0.05);治疗后6个月、12个月两组患者组间差异有统计学意义(P<0.05),VAS评分、影像学评分随时间均呈下降趋势,其中外敷组下降趋势更明显。VAS评分存在时间-分组交互效应(P<0.01)在治疗前以及治疗后3个月、6个月两组患者组间差异无统计学意义(P>0.05),在治疗12个月后差异有统计学意义(P<0.05),影像学评分存在时间-分组交互效应(P<0.01),在治疗前以及治疗后3个月两组患者组间差异无统计学意义(P>0.05),在治疗后6个月、12个月两组患者组间差异有统计学意义(P<0.05),治疗12个月后外敷组的优良率为72.5%,优于对照组的36.8%(P<0.05),各组疗效评分在治疗12个月后均优于同组治疗前,差异有统计学意义(P<0.001)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:健脾外敷法联合仙灵骨葆胶囊治疗早期酒精性股骨头坏死疗效安全可靠,能够明显改善患者髋关节功能,减轻疼痛,延缓股骨头塌陷等功能,提高患者生活质量。
Objective:To observe the clinical efficacy and safety of Jianpi external application combined with Xianling Gubao capsule in early alcoholic femoral head necrosis.Methods:A total of 80 patients with early alcoholic femoral head necrosis diagnosed and treated in the Affiliated Sports Hospital of Chengdu Sport University were randomly divided into the external application group and the control group,with 40 columns in each group.Patients in the control group were treated with oral administration of Xianlinggubao capsule,lower limb traction and hip joint functional exercise,while patients in the external application group were treated with external application of Jianpi external application prescription on the basis of the treatment in the control group.The Harris hip score,visual analogue scale(VAS) and imaging score of the hip joint of the two groups were compared before treatment and 3,6 and 12 months after treatment,respectively.The clinical efficacy of the two groups was compared after treatment,and the adverse reactions were recorded.Results:During the treatment,the Harris scores of the two groups showed an upward trend over time.There was time-group interaction effect(P<0.05),in which the upward trend in the external application group was more obvious,and there was no significant difference between the two groups before treatment and 3 months after treatment(P>0.05).Six months and 12 months after treatment,the differences between the two groups were statistically significant(P<0.05),12 months after treatment,and 6 months and 12 months after treatment,the differences between the two groups were statistically significant(P<0.05).Conclusion:Jianpi external application combined with Xianlinggubao capsule in the treatment of early alcoholic femoral head necrosis.
