摘要
目的:分析重症急性胰腺炎(SAP)患者发生院内感染的危险因素,并探究其护理对策。方法:对河南宏力医院2018年1月至2020年8月期间收治的75例急性SAP患者进行回顾性分析,调查患者临床资料,记录医院感染发生情况,分析患者发生院内感染的危险因素,并制定针对性的护理干预措施。结果:75例患者中有10例发生感染,发生率为13.33%,均为呼吸道感染。多因素logistic回归分析结果显示,合并糖尿病、进行机械通气、进行深静脉置管、留置腹腔引流管、留置导尿管、器官衰竭个数多、禁食时间长和改良计算机断层扫描严重程度指数(MCTSI)评分高均是SAP患者发生院内感染的独立危险因素(P<0.05)。结论:SAP患者院内感染发生率较高,糖尿病、多项医源性侵入性操作、禁食时间长、MCTSI评分高等因素与患者发生院内感染密切相关,临床中应该加强对患者的防护,以避免感染发生。
Objective To analyze the risk factors of nosocomial infection in patients with severe acute pancreatitis(SAP)and explore its nursing countermeasures.Methods A retrospective analysis of 75 patients with acute SAP admitted to Henan Hongli Hospital from January 2018 to August 2020 was conducted to investigate the clinical data of the patients,record the occurrence of nosocomial infection,analyze the risk factors of nosocomial infection,and formulate targeted nursing interventions.Results Infection occurred in 10 of 75 patients(13.33%).All were respiratory tract infections.Multivariate logistic regression analysis showed that diabetes mellitus,mechanical ventilation,deep vein catheterization,abdominal drainage tube indentation,indentation catheter,organ failure number,long fasting time and high modified computed tomography severity index(MCTSI)score were all independent risk factors for nosocomial infection in SAP patients(P<0.05).Conclusion The incidence of nosocomial infection in SAP patients is high,diabetes,multiple iatrogenic invasive operations,long fasting time,and high MCTSI score are closely related to the occurrence of nosocomial infection in patients.In clinical practice,the protection of patients should be strengthened to avoid infection occurs.
