摘要
目的分析老年心功能不全患者衰弱现状及相关因素,使临床人员及早识别老年人衰弱。方法运用便利抽样法,选取2021年11月—2022年1月芜湖市某三级甲等医院老年科和心血管内科住院的150例老年心功能不全患者作为调查对象,使用一般资料调查表、FRALL衰弱筛查量表、微型营养评估量表简表(MNA-SF)、Barthel指数评定量表、简版老年抑郁量表(GDS-15)及实验室相关指标等进行问卷调查。应用多元Logistic回归分析老年心功能不全住院患者衰弱相关危险因素。结果老年心功能不全患者未出现衰弱为48例(32%),衰弱前期为34例(45.3%),出现衰弱68例(22.7%);多元Logistic回归分析结果显示:基本日常生活能力、营养状况、钠脑肽(Brain natriuretic peptide,BNP)为老年心功能不全患者衰弱独立影响因素(P<0.05)。结论老年心功能不全住院患者衰弱发生率较高,临床人员应加强对衰弱老年人的关注度,尤其针对生活自理能力不足、存在营养风险的病人,积极采取针对性相关措施改善病人生存质量、降低再入院率、延长患者寿命。
Objective To analyze the current status of frailty and related factors in elderly patients with cardiac insufficiency,and to enable clinical staff to identify frailty in the elderly as early as possible.Methods Using the convenience sampling method,150 elderly patients suffered with cardiac insufficiency who were hospitalized in the geriatric department and cardiovascular medicine department of a grade 3 hospital in Wuhu city from November 2021 to January 2022 were selected as the survey objects.General information questionnaire,FRALL frailty screening scale,mini-nutrition assessment scale short form(MNA-SF),Barthel index rating scale,geriatric depression scale short(GDS-15)and laboratory-related indicators were used for questionnaire survey.Multivariate logistic regression was used to analyze the risk factors of frailty in elderly hospitalized patients with cardiac insufficiency.Results Among the elderly patients with cardiac insufficiency,48 cases(32%)did not present frailty,34 cases(45.3%)had frailty of early stage,and 68 cases(22.7%)had frailty.Multivariate logistic regression analysis showed that basic daily living ability,nutrition status,and brain natriuretic peptide(BNP)were independent influencing factors of frailty in elderly patients with cardiac insufficiency(P<0.05).Conclusions The incidence of frailty is high in elderly hospitalized patients with cardiac insufficiency.Clinical staff should pay more attention to the frail elderly,especially for patients with insufficient self-care ability and nutritional risk,and actively take targeted measures toimprove the quality of life of patients,reduce the readmission rate,and prolong the life of patients.
