摘要
目的系统评价针刀联合推拿治疗腰椎间盘突出症的疗效。方法计算机检索中国知网数据库(CNKI)、万方数据库(WanFang)、维普数据库(VIP)、PubMed、The Cochrane Library、Web of Science,检索时间限定为2010年1月1日-2020年12月30日,收集针刀联合推拿治疗腰椎间盘突出症的随机对照实验,采用RevMan5.3软件进行Meta分析,根据Cochrane偏倚风险评估工具对纳入文献进行质量评价。结果纳入7篇随机对照试验,共750例患者,其中试验组375例,对照组375例。试验组总有效率[RR=1.18,95%CI(1.11,1.25),P<0.00001]高于对照组,M-JOA积分[MD=-6.10,95%CI(-8.15,-4.04),P<0.00001]和VAS评分[MD=-1.46,95%CI(-1.66,-1.25),P<0.00001]优于对照组,肿瘤坏死因子-α[MD=-0.29,95%CI(-0.33,-0.25),P<0.00001]和白介素-1β[MD=-0.09,95%CI(-0.11,-0.06),P<0.00001]低于对照组,差异有统计学意义(P<0.05)。结论针刀联合推拿治疗能更好地减轻腰椎间盘突出症的疼痛症状,改善M-JOA积分、致炎因子水平,提高临床疗效,但今后还需高质量研究来证实。
Objective To systematically evaluate the efficacy of needle knife combined with massage in the treatment of lumbar disc herniation.Methods The CNKI,WanFang,VIP,PubMed,The Cochrane Library,and Web of Science were searched by computer.The retrieval time was limited to January 1,2010–December 30,2020.The randomized controlled trials of acupuncture and knife combined with massage in the treatment of lumbar disc herniation were collected.The RevMan5.3 software was used for Meta-analysis,and the quality of the included literature was evaluated according to the Cochrane bias risk assessment tool.Results Seven randomized controlled trials were included,with a total of 750 patients,including 375 patients in the experimental group and 375 patients in the control group.The total effective rate of the experimental group[RR=1.18,95%CI(1.11,1.25),P<0.00001]was higher than that of the control group,the M-JOA score[MD=-6.10,95%CI(-8.15,-4.04),P<0.00001]and VAS score[MD=-1.46,95%CI(-1.66,-1.25),P<0.00001]were better than those of the control group,tumor necrosis factor-α[MD=-0.29,95%CI(-0.33,-0.25),P<0.00001]and interleukin-1β[MD=-0.09,95%CI(-0.11,-0.06),P<0.00001]were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Needle knife combined with massage therapy can better relieve the pain symptoms of lumbar disc herniation,improve the M-JOA score and the level of inflammatory factors,and improve the clinical efficacy.However,high-quality studies are needed to confirm it in the future.
