伴自杀行为和不伴自杀行为精神分裂症睡眠脑电功率谱密度比较研究 被引量:5

摘要 目的比较伴自杀行为和不伴自杀行为精神分裂症患者睡眠及脑电功率谱密度差异。方法纳入近1个月内出现自杀行为和无自杀行为的精神分裂症患者各20例,采用失眠严重程度指数量表、睡眠信念与态度量表、梦魇严重程度指数、Epworth嗜睡量表评估主观睡眠,采用多导睡眠监测评估客观睡眠,通过睡眠脑电滤波器分析和处理脑电功率谱密度,并对比两组患者的主观睡眠、客观睡眠指标及睡眠脑电功率谱密度差异。结果伴自杀行为精神分裂症和不伴自杀行为精神分裂症主观睡眠量表评分和客观睡眠指标无统计学差异(P>0.05)。伴自杀行为精神分裂症组较不伴自杀行为精神分裂症组清醒期(W期)、N1期、N2期、N3期、快速眼动期(R期)α波或β波功率谱密度减弱(P<0.05),伴自杀行为精神分裂症组W期、N1期、N2期、N3期、R期δ波或θ波功率谱密度较不伴自杀行为精神分裂症组增强(P<0.05),其余功率谱密度两组间无统计学差异(P>0.05)。结论伴自杀行为精神分裂症患者睡眠各期α波或β波(快波)功率谱密度减弱,δ波或θ波(慢波)功率谱密度增强。 Objective To compare the sleep and EEG power spectral density of schizophrenic patients with and without suicidal behavior.Methods Twenty schizophrenia patients with suicidal behavior and twenty schizophrenia patients without suicidal behavior were selected.Subjective sleep was assessed by Insomnia Severity Index Scale(ISI),Dysfunctional Belief and Attitude about Sleep(DBAS),Disturbing Dreams and Nightmare Severity Index(DDNSI)and Epworth Sleepiness Scale(ESS).Objective sleep was assessed by polysomnography monitoring,and power spectral density was analyzed and processed by sleep EEG filter.The differences of subjective sleep,objective sleep and sleep EEG power spectral density were compared between the two groups.Results There was no significant difference in subjective sleep and objective sleep between schizophrenia with suicidal behavior and schizophrenia without suicidal behavior(P>0.05).Power spectral density ofαwave orβwave in W,N1,N2,N3 and R stages in schizophrenia with suicidal behavior was significantly weaker than in schizophrenia without suicidal behavior(P<0.05).Power spectral density ofδwave orθwave in W,N1,N2,N3 and R stages was significantly higher in schizophrenia with suicidal behavior than in schizophrenia without suicidal behavior(P<0.05).There was no significant difference in other power spectral densities between the two groups(P>0.05).Conclusions Power spectral density ofαWave orβ(fast wave)in sleep stages is significantly reduced in schizophrenic patients with suicidal behavior than without suicidal behavior,power spectral density ofδwave orθwave(slow wave)in Sleep stages is increased significantly in schizophrenic patients with suicidal behavior than without suicidal behavior.
