摘要
目的:观察温肾通络汤联合头皮针长留针法对卒中后认知障碍(PSCI)患者的临床疗效。方法:将2019年9月-2021年9月泰安市中医医院康复科76例PSCI患者随机分为观察组和对照组,各38例,两组均接受常规药物治疗、基础康复治疗及头皮针长留针法治疗,另外,对照组口服吡拉西坦片,观察组给予温肾通络汤治疗,疗程均为8周。比较两组患者治疗后的临床效果。结果:治疗8周后,两组患者蒙特利尔认知评估量表(MoCA)评分、神经行为认知状态检查量表(NCSE)评分、改良Barthel指数(MBI)评分和脑动脉血流平均流速(Vm)均较治疗前明显提高(P<0.05),脑动脉搏动指数(PI)均较治疗前降低(P<0.05),且观察组各指标改善幅度均优于对照组(P<0.05)。两组治疗期间均无不良反应发生。结论:温肾通络汤联合头皮针长留针治疗可显著提高PSCI患者的认知功能及日常生活能力,还可增加脑灌流,且安全性较高。
Objective:To observe the effect of Wenshen Tongluo Decoction combined with scalp acupuncture for long time therapy on post-stroke cognitive impairment.Method:A total of 76 patients with post-stroke cognitive impairment from September 2019 to September 2021 in Department of Rehabilitation,Tai’an Hospital of Traditional Chinese Medicine were randomly divided into the observation group and the control group,with 38 cases in each group,both groups were treated with routine drug therapy,basic rehabilitation therapy and scalp acupuncture for long time therapy,in addition,the control group was treated with Piracetam,and the observation group was treated with Wenshen Tongluo Decoction,the course of treatment was 8 weeks.The clinical efficacy of the two groups were analyzed after the treatment.Result:After 8 weeks of treatment,Montreal cognitive assessment(MoCA)score,neurobehavioral cognitive status examination(NCSE)score,modified Barthel index(MBI)score,and the average flow velocity(Vm)in two groups were significantly improved(P<0.05),and the pulsatility index(PI)was significantly decreased(P<0.05),the range of improvement of each index in the observation group were more significantly than those in the control group(P<0.05).No adverse reactions occurred in the two groups during treatment.Conclusion:Wenshen Tongluo Decoction combined with scalp acupuncture for long time therapy in treating post-stroke cognitive impairment can significantly improve the patients’cognitive function and ability of daily living,and improve the patient’cerebral perfusion,the safety is higher.
