摘要
目的探讨胫骨骨折患者非扩髓髓内钉治疗后用Johner-Wruhs标准评价预后效果的应用。方法选择2020年12月—2021年5月在甘肃省酒钢医院治疗的胫骨中下段骨折患者15例,术前进行全身检查制定手术方案,患者经闭合复位髓内钉术式治疗后对其进行随访,记录术中指标和末次随访的Johner-Wruhs等级。结果15例患者在手术中均未出现血管神经损伤,手术时间40~90 min,术中失血量为35~80 mL,皮肤切口5~12 cm,平均住院日在5~20 d。所有患者均进行随访,随访时间为5~18个月。Johner-Wruhs等级均为优良级,进一步分析发现,性别、年龄和骨折分型在评价等级中差异无统计学意义(P=0.622、0.525、0.640)。结论用闭合复位髓内钉术式对胫骨骨折患者进行手术后,采用Johner-Wruhs评价标准对患者进行随访,患者的治疗效果均能达到优良。
Objective To investigate the application of the Johner-Wruhs criteria to evaluate the prognostic outcome after non-expanded intramedullary nailing treatment in patients with tibial fractures.Methods 15 patients with middle and lower tibial fractures treated in Jiugang Hospital of Gansu from December 2020 to May 2021 were selected for preoperative whole-body examination to develop the surgical plan.Patients were followed up after treatment with closed reduction intramedullary nailing,and intraoperative indices and Johner-Wruhs grade at the last follow-up were recorded.Results All 15 patients had no vascular nerve injury during surgery,and the operative time was 40-90 min,with intraoperative blood loss of about 35-80 mL,skin incision of 5-12 cm,and average hospital stay of 5-20 d.All patients were followed up for a period of 5 to 18 months.Johner-Wruhs grades were all excellent,and further analysis revealed that gender,age,and the difference of fracture typing were not statistically significant in the evaluation grades(P=0.622,0.525,0.640).Conclusion After operating on patients with tibial fractures using the closed reduction intramedullary nailing approach,patients were followed up using the Johner-Wruhs evaluation criteria,and all patients were able to achieve excellent outcomes.
