摘要
目的探讨炎症细胞与不同类型的下肢深静脉血栓形成(LDVT)之间的关系。方法回顾性分析2013年1月至2017年12月郑州大学第一附属医院3776例诊断为LDVT患者的临床资料,根据血栓的解剖部位分为四组:1311例肌间静脉型,1528例周围型除外孤立型肌间静脉血栓,208例中央型和729例混合型,另选323例健康成人作为对照组,比较各组的白细胞、中性粒细胞、淋巴细胞和单核细胞数据。结果白细胞、中性粒细胞、淋巴细胞和单核细胞在LDVT患者和健康成年人群之间的分布,差异有统计学意义(P<0.05)。与健康成年人群相比,LDVT患者血清中的白细胞、中性粒细胞和单核细胞水平均升高,而淋巴细胞水平降低。中性粒细胞和淋巴细胞与LDVT的血栓解剖部位存在相关性,中性粒细胞每增加一个单位,患者发生中央型和混合型LDVT的风险是肌间静脉型的0.667倍(95%CI:0.472~0.944,P<0.05)和0.704倍(95%CI:0.544~0.911,P<0.05);淋巴细胞每增加一个单位,患者发生中央型和混合型LDVT的风险是肌间静脉型的0.582倍(95%CI:0.376~0.902,P<0.05)和0.632倍(95%CI:0.465~0.859,P<0.05)。结论中性粒细胞和淋巴细胞水平高低可能与LDVT的类型有相关性,对临床上的早期诊断、治疗和评估预后具有一定的参考价值。
Objective To investigate the relationship between inflammatory cells and different types of deep venous thrombosis of lower extremity(LDVT).Methods The clinical data of 3776 patients diagnosed with LDVT in our hospital from Jan.2013 to Dec.2017 were retrospectively analyzed,and patients were divided into four groups according to the anatomical site of thrombus:1311 cases of isolated intermuscular venous thrombosis,1528 cases of peripheral type except isolated intermuscular venous thrombosis,208 cases of central type and 729 cases of mixed type.Besides,323 healthy adults were enrolled as control group.The data of white blood cells,neutrophils,lymphocytes and monocytes in each group were compared.Results The distribution of white blood cells,neutrophils,lymphocytes and monocytes between LDVT patients and healthy adults was statistically significant(P<0.05).Compared with healthy adults,serum levels of white blood cells,neutrophils,and monocytes were increased and level of lymphocytes was decreased in patients with LDVT.Neutrophils and lymphocytes had a correlation with the anatomic location of thrombus of LDVT.For each additional unit of neutrophils,the risk of central LDVT and mixed LDVT were 0.667(95%CI:0.472-0.944,P<0.05)and 0.704(95%CI:0.544-0.911,P<0.05)times higher than isolated intermuscular venous thrombosis in patients.For each additional unit of lymphocytes,the risk of central LDVT and mixed LDVT were 0.582(95%CI:0.376-0.902,P<0.05)and 0.632(95%CI:0.465-0.859,P<0.05)times higher than isolated intermuscular venous thrombosis in patients.Conclusion The level of neutrophils and lymphocytes may correlate with the type of LDVT,which provide some reference value for early diagnosis,treatment and evaluation of prognosis.
