选择性肝血管阻断的二次肝切除术治疗肝癌的临床研究进展

摘要 目前,对于剩余肝脏体积不达标的肝癌病人,手术策略是通过诱导刺激健侧肝脏增生和病侧肝脏萎缩来实现二次肝切除。近年来,联合肝脏离断及门脉结扎的二次肝切除术(associating liver partition and portal vein ligation for staged hepatectomy,ALPPS)的问世为解决这一难题提供了新的办法。随着相关研究的不断进展,在常规ALPPS的基础上出现了多种改良术式,其中选择性肝血管阻断的方法,因具有比传统ALPPS安全性更佳、抑制肿瘤生长效果更好以及更易推广等优势,在医学界广受关注。 For hepatic carcinoma patients with substandard future liver remnant(FLR),conventional surgical strategy has been inducing and stimulating healthy side′s liver to proliferate and diseased side′s liver to shrink to achieve a second liver resection.Recent advent of associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)may be regarded as a solution to this problem.With continuous advances of related researches,a large variety of modified procedures have appeared on the basis of conventional ALPPS.Among them,secondary hepatectomy with selective hepatic vascular occlusion is much safer and more effective in inhibiting tumor growth than ALPPS.Its readiness for a wider popularization has attracted great attention in clinical practices.
