摘要
目的 分析急性脑梗死患者血清中HMGB、APN、oxLDL表达水平与颈动脉粥样硬化的相关性。方法 选取2018年3月-2020年12月进行体检的健康者60例设为对照组,选取同期收治的急性脑梗死患者60例设为研究组,通过颈动脉超声检查结果,根据不同斑块性质将其分为内膜粗糙组(n=21)、稳定斑块组(n=25)、不稳定斑块组(n=14)。比较两组患者血清中高迁移率族蛋白(HMGB)、脂联素(APN)、氧化低密度脂蛋(oxLDL)表达水平,不同斑块性质患者的血清HMGB、oxLDL、APN水平,分析血清HMGB、oxLDL、APN水平与颈动脉粥样硬化的相关性。结果 治经对比,研究组患者血清HMGB、oxLDL水平均高于对照组,APN水平低于对照组,差异均具有统计学意义(P<0.05);颈动脉粥样硬化程度越高,HMGB、oxLDL水平越高,APN水平越低,差异均具有统计学意义(P<0.05);HMGB、oxLDL水平与颈动脉粥样硬化均呈正相关(r=0.504、0.533,P<0.05);APN水平与颈动脉粥样硬化均呈负相关(r=-0.602,P<0.05)。结论 急性脑梗死患者血清HMGB、oxLDL水平与颈动脉粥样硬化呈正相关,与APN水平呈负相关,三者在评估急性脑梗死患者颈动脉粥样硬化程度方面具有一定的临床意义。
Objective To analyze the correlation between the expression levels of HMGB, APN and oxLDL in serum and carotid atherosclerosis in patients with acute cerebral infarction. Methods 60 healthy subjects who underwent physical examination in our hospital from March 2018 to December 2020 were selected as the control group, and 60 patients with acute cerebral infarction treated in our hospital in the same period were selected as the study group. According to the results of carotid ultrasound, they were divided into rough inner membrane group(n=21), stable plaque group(n=25) and unstable plaque group(n=14) according to different plaque properties. The expression levels of high mobility group protein(HMGB), adiponectin(APN) and oxidized low density lipoprotein(oxLDL) in serum and the levels of HMGB, oxLDL and APN in patients with different plaque properties were compared between the two groups. The correlation between the levels of HMGB, oxLDL and APN and carotid atherosclerosis was analyzed. Results The levels of serum HMGB and oxLDL in the study group were higher than those in the control group, and the level of APN was lower than that in the control group(P<0.05);The higher the degree of carotid atherosclerosis, the higher the levels of HMGB and oxLDL, and the lower the level of APN, the difference was statistically significant(P<0.05);The levels of HMGB and oxLDL were positively correlated with carotid atherosclerosis(r=0.504,0.533, P<0.05);The level of APN was negatively correlated with carotid atherosclerosis(r=-0.602,P<0.05). Conclusions The levels of serum HMGB and oxLDL in patients with acute cerebral infarction are positively correlated with carotid atherosclerosis and negatively correlated with APN. They have certain clinical significance in evaluating the degree of carotid atherosclerosis in patients with acute cerebral infarction.
