胰激肽原酶结合非诺贝特对糖尿病肾病患者血清S1P、LXA4表达的影响 被引量:4

Effects of Pancreatic Kinkinase Combined with Fenofibrate on Serum S1Pand LXA4 Expressions in Patients with Diabetic Nephropathy

摘要 目的胰激肽原酶结合非诺贝特对糖尿病肾病(diabetic nephropathy,DN)患者血清1-磷酸鞘氨醇(sphingosine-1-phosphate,S1P)、脂氧素A4(lipid oxygen element A4,LXA4)表达的影响。方法选取2019年10月至2020年10月在我院就诊的102例DN患者为研究对象,按照随机数表法分为对照组和观察组,每组51例,对照组给予非诺贝特治疗,观察组在对照组的基础上联合胰激肽原酶治疗,观察两组患者的临床疗效,治疗前后两组患者的肾功能、凝血功能、血流动力学指标、血清学指标变化情况。结果观察组患者的治疗总有效率高于对照组(P<0.05);治疗前两组患者的各项肾功能指标差异无统计学意义(P>0.05),治疗后,两组患者的各项肾功能指标均下降,且观察组下降幅度更大(P<0.05);两组患者治疗前TT、APTT、FIB水平差异无统计学意义(P>0.05),治疗后两组患者TT、APTT值均升高,且观察组高于对照组;FIB值均降低,且观察组低于对照组(P<0.05);治疗前两组患者全血高切黏度、全血中切黏度、全血低切黏度、PVC、ESR、血流方程K值、红细胞聚集指数、血浆黏度等指标值差异无统计学意义(P>0.05);治疗后各指标值均降低,与对照组相比,观察组患者全血低切黏度、血流方程K值、红细胞聚集指数、血浆黏度值显著降低(P<0.05);两组患者治疗前S1P、LXA4水平差异均无统计学意义(P>0.05);治疗后两组SIP、LXA4水平升高,且观察组高于对照组(P<0.05)。结论胰激肽原酶结合非诺贝特治疗糖尿病肾病可改善患者的肾功能、凝血功能,有助于血流动力学稳定,可升高血清S1P、LXA4水平。 Objective To investigate effects of pancreatic kininogenase combined with fenofibrate on expressions of serum sphingosine 1-phosphate and lipoxygenin A4 in patients with diabetic nephropathy.Methods One hundred and two DN patients visited our hospital from October,2019 to October,2020 were selected for the study,and were divided into the control group and observation group according to the random number table method,with 51 cases in each group.The control group was given fenofibrate treatment,while the observation group was combined with pancreatic kininogenase treatment in addition.The clinical efficacy of the two groups was observed,as well as changes of renal function,coagulation function,hemodynamic index,and serological index in both groups before and after treatment.Results The total efficiency of treatment was higher in the observation group than in the control group(P<0.05);there was no significant difference in the renal function indexes between two groups before treatment(P>0.05),and after treatment,the renal function indexes decreased in both groups,and the decrease was greater in the observation group(P<0.05).There was no significant difference in TT,APTT and FIB levels between the two groups before treatment(P>0.05),and after treatment,there were no significant differences in TT,APTT and FIB levels between the two groups before treatment(P>0.05),but after treatment,TT and APTT values increased in both groups and were higher in the observation group than in the control group.FIB values decreased in both groups and were lower in the observation group than in the control group(P<0.05).There were no significant differences in the index values of whole blood high cut viscosity,whole blood medium cut viscosity,whole blood low cut viscosity,PVC,ESR,K value of blood flow equation,erythrocyte aggregation index and plasma viscosity between the two groups before treatment(P>0.05);after treatment,all index values decreased and were different from those of the control group.After treatment,all index values decrea
