摘要
目的观察活血接骨汤对股骨粗隆间骨折患者防旋型股骨近端髓内钉内固定术(PFNA)术后骨折愈合的影响。方法选取南通市通州区中医院骨伤科收治的股骨粗隆间患者49例患者,依据随机分配法分为观察组26例,对照组23例。2组均行PFNA术,观察组在术后常规治疗基础上口服活血接骨汤,对照组在术后常规治疗基础上口服钙尔奇钙片抗骨质疏松治疗,记录2组术后第3个月骨折愈合情况及术后1 d、术后3个月血液流变学相关指标(血沉、血浆黏度、全血比黏度、红细胞压积)及骨代谢相关指标[生长激素(hGH)、血清骨钙素(OC)、Ⅰ型胶原羧基末端肽(CTX)、骨碱性磷酸酶(BALP)、甲状旁腺激素(PTH)]。结果术后3个月时观察组愈合率为92.31%,对照组愈合率为86.96%,2组对比差异无统计学意义(P>0.05)。与术后1 d相比,2组术后3个月的血液流变学相关指标及PTH水平均降低,hGH、OC、CTX、BALP均升高(均P<0.05)。术后3月观察组血沉、全血比黏度、红细胞压积及PTH水平均低于对照组(均P<0.05),hGH、OC、CTX、BALP水平均高于对照组(P均<0.05)。结论活血接骨汤有助于股骨粗隆间骨折PFNA术后患者骨折的愈合,其机制可能与骨折断端血液供应及骨代谢的改善相关。
Objective To observe the effect of Huoxue Jiegu decoction on fracture healing in patients with intertrochanteric fracture after proximal femoral nail antirotation(PFNA).Methods 49 cases of intertrochanteric patients in Nantong Tongzhou Hospital of Traditional Chinese Medicine were randomly divided into observation group(26 cases)and control group(23 cases).The observation group was treated with Huoxue Jiegu decoction on the basis of conventional treatment after operation,and the control group was treated with calcium tablets on the basis of conventional treatment after operation.The fracture healing at the third month after operation,the hemorheology related indexes(ESR,Plasma Viscosity,Whole Blood Specific Viscosity,Hematocrit)and bone metabolism related indexes at the first day and the third month after operation were recorded Indicators(hGH,OC,CTX,BALP,PTH).Results The healing rate of the observation group was 92.31%,and that of the control group was 86.96%.There was no significant difference between the two groups(P>0.05).Compared with 1 day after operation,hemorheology related indexes and PTH levels of the two groups were decreased,hGH,OC,CTX and BALP were increased at 3months after operation(allP<0.05).At 3 months after operation,ESR,whole blood viscosity,hematocrit and PTH levels in the observation group were lower than those in the control group(allP<0.05),and HGH,OC,CTX and BALP levels in the observation group were higher than those in the control group(allP<0.05).Conclusion Huoxue Jiegu decoction is helpful to the fracture healing of patients with intertrochanteric fracture after PFNA,and its mechanism may be related to the improvement of blood supply and bone metabolism.
