美学区种植过渡修复体对种植体周围软组织的调控 被引量:3

摘要 在美学区种植修复中,合理运用种植过渡修复体,通过保持、引导或调整种植体周围软组织,可以获得良好的软组织美学效果。过渡修复体的穿龈形态是除外科手段以外,对种植体周围软组织起到调控作用的重要因素之一。根据位置不同,过渡修复体的穿龈形态分为龈缘至龈下1 mm的关键区域和龈下1 mm至种植体平台的次关键区域两个部分。关键区域需要有效地支撑龈缘,直接影响修复体龈缘位置、形态;次关键区域在多数情况下需要呈微凹形态,为软组织生长提供空间,保证软组织厚度,而在少部分情况下则需要略增加凸度挤压和支撑种植体周围软组织。在长期的临床实践中,过渡修复体可以通过直接或间接、传统或数字化等多种方式制作。本文旨在总结不同临床条件下的过渡修复体穿龈形态设计,过渡修复体的制作方法,及其对种植体周围软组织的作用,以供临床参考。 Proper management of the interim implant-supported restoration has become an important method to keep,guide or adjust the peri-implant soft tissue,making for an esthetic result of the soft tissue.A key factor of controlling the peri-implant soft tissue after all the surgery procedures is the emergence profile of the interim implant-supported restoration.It can be identified by two distinct zones:the critical zone and the subcritical zone.The critical zone is the sectional contour of the restoration from the gingival margin to 1 mm below.The support of the critical zone has a close influence on the position and geometry of the gingival margin of the restoration.The subcritical zone is the sectional contour of the restoration from 1 mm below the margin to the implant platform.Usually,the subcritical zone is designed in a concave profile to provide space for the soft tissue and to guarantee the thickness of the soft tissue.Occasionally it can also be designed in a slightly convex profile to press and support the soft tissue in some special situations.The interim implant-supported restoration can be fabricated by direct or indirect,traditional or digital methods.The aim of this paper is to discern the characteristics of the emergence profile of the interim implant-supported restoration in different situations and to introduce the fabrication methods to clinical practice.
