摘要
目的:通过对肝门周围胆管癌术后并发症的分析,从而避免并发症的发生,达到防治的目的。方法:回顾性分析2017年7月至2020年7月赣南医学院第一附属医院收治的21例进行手术治疗的肝门周围胆管癌患者临床资料,分析肝门周围胆管癌术后并发症的防治。结果:21例进行手术治疗的肝门周围胆管癌患者中,根治性切除率为61.9%,姑息性手术比率为38.1%,术后病理提示胆管细胞腺癌。术后并发症情况:共发生13例次,胆漏4例次(30.8%),腹腔出血1例次(7.6%),腹腔感染4例次(30.8%),消化道出血2例次(15.4%),肝功能衰竭2例次(15.4%),无围手术期死亡病例。住院时间(27.4±8.7)天。结论:随着肝门周围胆管癌手术方式的不断扩大,患者的生存率得到了提高,但术后并发症也明显增多。采取针对性的干预措施,对降低肝门周围胆管癌术后并发症发生率具有重要作用。
Objective:To analyze the postoperative complications of perihilar cholangiocarcinoma in order to avoid the occurrence of complications and achieve the purpose of prevention and treatment.Methods:Clinical data of 21 cases with perihilar cholangiocarcinoma who received surgical treatment in The First Affiliated Hospital of Gannan Medical University from July 2017 to July 2020 were retrospectively analyzed,and the prevention and treatment of postoperative complications of perihilar cholangiocarcinoma were analyzed.Results:In 21 cases of perihilar cholangiocarcinoma,radical resection rate was 61.9%,palliative surgery rate was 38.1%,postoperative pathology indicated bile duct cell adenocarcinoma.Postoperative complications:a total of 13 cases occurred,including 4 cases of biliary leakage(30.8%),1 case of abdominal bleeding(7.6%),4 cases of abdominal infection(30.8%),2 cases of gastrointestinal bleeding(15.4%),2 cases of liver failure(15.4%),and no perioperative death.The length of hospital stay was with an average of 27.4±8.7 days.Conclusion:With the expansion of perihilar cholangiocarcinoma surgery,the survival rate of patients has been improved,but the postoperative complications are also significantly increased.Taking targeted intervention measures plays an important role in reducing the incidence of postoperative complications in patients with perihilar cholangiocarcinoma.
