摘要
目的:整理挖掘依托古今医案云平台(V2.3.5)中医药治疗痛风性关节炎的用药规律,为临床组方治疗及中药复方开发提供新思路。方法:应用古今医案云平台检索到的2006年1月至2019年12月治疗痛风性关节炎有效医案,标准化处理后进行中药频次、属性分析、关联分析、聚类分析及多尺度的backbone复杂网络分析。结果:共纳入122个医案,涉及中药249味。频次前5的中药为土茯苓、黄柏、薏苡仁、粉萆薢、苍术。药性以温、平、寒居多,药味以甘、苦为首,归经以肝、脾、胃为主。得到9个核心药对,聚类分析2个组合,核心处方为苍术、黄柏、土茯苓、粉萆薢、薏苡仁、威灵仙、甘草、忍冬藤、泽泻、牛膝。结论:现代中医药认为湿、热、痰、瘀、虚是引起痛风的主要病因,治疗该病多从肝、胃、脾三脏出发,以甘、苦、辛味药物为主,注重清热、利湿、活血止痛等药物的应用,体现了“清热祛湿治标”“化瘀通络活血止痛”“益肾固其本”的治疗原则。
Objective To explore the regularity of TCM treatment of gouty arthritis in the past thirteen years using Gu-Jin medical case cloud platform V2.3.5 and provide new ideas for its treatment.Methods The effective medical cases in the treatment of gouty arthritis from January 2006 to December 2019 in the Gu-Jin medical case cloud platform were applied.After standardized processing,the frequency,attribute analysis,association analysis,cluster analysis and multi-scale backbone complex network analysis were carried out.Results A total of 122 internal medicines were collected,including 249 traditional Chinese medicines,The top 5 traditional Chinese medicines in frequency were Rhizoma smilacis Glabrae、Cortex Phellodendri.Semen Coicis、Rhizome Atractylodis.Qi is mainly calm warm and cold,and the taste is sweet and bitter.The main meridians are liver,spleen and stomach.Nine core drug pairs were obtained,and two combinations were analyzed by cluster analysis,and The core prescriptions are Rhizoma atractylodis,Cortex phellodendri,Rhizoma smilacis Glabrae,,Coix Seed,Weilingxien,Licorice,Honeysuckle stem,Alisma Alismata and Radix Achyranthes.Conclusion The main causes of gout are Dampness,heat,phlegm,blood stasis and deficiency.The treatment of the disease mainly starts from the liver,stomach and spleen,with sweet,bitter and spicy drugs,and pays attention to the application of drugs such as clearing heat,promoting dampness,promoting blood circulation and relieving pain,The treatment principles are“clearing heat and treating symptoms with clearing damp”,“removing blood stasis,dredging collaterals,promoting blood circulation and relieving pain”and“enriching kidney and strengthening its origin”.
