摘要
目的探讨直肠痔上黏膜环切术(PPH)联合分区剥扎断桥缝合术治疗重度环状混合痔患者的临床效果。方法将60例重度环状混合痔患者随机分为联合组和对照组,各30例。联合组给予PPH联合分区剥扎断桥缝合术治疗,对照组给予分区剥扎断桥缝合术治疗。比较两组患者的手术情况、术后疼痛视觉模拟量表(VAS)评分、手术治疗效果、术后并发症,以及术前和术后7 d焦虑自评量表(SAS)评分和36项健康调查简表(SF-36)评分。结果与对照组比较,联合组的住院时间、创面愈合时间均更短,术中出血量更少,术后各时间点的疼痛VAS评分和术后并发症总发生率均更低(均P<0.05)。联合组的手术治疗效果优于对照组,其中治愈率、显效率均高于对照组(均P<0.05)。术后7 d,联合组患者的SAS评分均低于术前和对照组,SF-36各项评分均高于术前,且生理职能、躯体疼痛、一般健康状况、社会功能、情感职能、精神健康评分均高于对照组(均P<0.05)。结论采用PPH联合分区剥扎断桥缝合术治疗重度环状混合痔患者可以优化手术相关指标,降低患者术后并发症发生率,减轻患者的疼痛和焦虑情绪,并提高患者生活质量,有利于患者术后恢复。
Objective To explore the clinical effects of procedure for prolapse and hemorrhoids(PPH)combined with region-divided stripping ligation and broken bridge suture for treating patients with severe annular mixed hemorrhoids.Methods A total of 60 patients with annular mixed hemorrhoids were randomly assigned to combination group or control group,with 30 cases in each group.The combination group was given PPH combined with region-divided stripping ligation and broken bridge suture for treatment,whereas the control group was treated with region-divided stripping ligation and broken bridge suture.The operative conditions,postoperative Visual Analogue Scale(VAS)score for pain,and therapeutic effects of operation,postoperative complications,as well as pre-and 7-day postoperative Self-Rating Anxiety Scale score,and 36-Item Short-Form Health Survey(SF-36)score in patients were compared between the two groups.Results Compared with the control group,the combination group interpreted shorter hospital stay,wound healing time,less intraoperative bleeding volume,and lower VAS score for pain at various time points after operation and total incidence rate of postoperative complications(all P<0.05).The combination group exhibited a superior clinical effect of operation to the control group,thereinto the cure rate and marked effective rate were higher in the combination group than in the control group(all P<0.05).Seven days after operation,patients in the combination group obtained lower SAS score as compared with before operation and the control group,higher scores of all items of the SF-36 than those before operation,and higher scores of psychological function,body pain,general health status,social function,emotional function,and mental health than those in the control group(all P<0.05).Conclusion Employing PPH combined with region-divided stripping ligation and broken bridge suture for treating severe annular mixed hemorrhoids can optimize the operation-related indices,decline the incidence rate of postoperative complications,reli
