摘要
目的探讨基于变革理论的集束化护理在胃癌患者全胃切除术后早期肠内营养支持中的应用效果。方法选取2019年1月至2021年2月我院收治的92例胃癌患者(均接受全胃切除术治疗)作为研究对象,采用随机数字表法将其分为对照组(46例,常规护理)与观察组(46例,常规护理+基于变革理论的集束化护理)。比较两组患者的机体功能康复时间、营养指标水平、并发症发生情况及护理满意度。结果观察组的肠鸣音恢复时间、肛门排气时间、开始进食时间、首次排便时间及住院时间均短于对照组,差异具有统计学意义(P<0.05)。护理后,观察组的血清前白蛋白、清蛋白、总蛋白水平均高于对照组,差异具有统计学意义(P<0.05)。观察组的并发症总发生率为10.87%,低于对照组的28.26%,差异具有统计学意义(P<0.05)。观察组的护理满意度为93.48%,高于对照组的65.22%,差异具有统计学意义(P<0.05)。结论基于变革理论的集束化护理应用于胃癌患者全胃切除术后早期肠内营养支持中的效果显著,可有效缩短患者机体功能康复时间,改善机体营养状态,降低并发症发生风险,提高护理满意度,值得临床推广应用。
Objective To explore the application effect of cluster nursing based on change theory on early enteral nutrition support in patients with gastric cancer after total gastrectomy.Methods A total of 92 patients with gastric cancer(all received total gastrectomy)who were treated in our hospital from January 2019 to February 2021 were selected as the research objects,and the patients were divided into control group(46 cases,routine nursing)and observation group(46 cases,routine nursing+cluster nursing based on change theory)by random number table method.The recovery time of body function,nutritional indexes levels,complications and nursing satisfaction were compared between the two groups.Results The recovery time of bowel sounds,anal exhaust time,start eating time,first defecation time and hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After nursing,the levels of serum prealbumin,albumin and total protein in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was 10.87%,which was lower than 28.26%in the control group,and the difference was statistically significant(P<0.05).The nursing satisfaction of the observation group was 93.48%,which was higher than 65.22%in the control group,and the difference was statistically significant(P<0.05).Conclusion The cluster nursing based on change theory applied in early enteral nutrition support for patients with gastric cancer after total gastrectomy has a significant effect,it can effectively short the recovery time of patients'body functions,improve the bodys'nutritional status,reduce the risk of complications,and improve nursing satisfaction,which is worthy of clinical promotion and application.
