摘要
目的探讨右美托咪定在非体外循环下心脏不停跳冠状动脉搭桥术中的应用效果。方法将2020年11月至2021年3月我院收治的83例冠心病患者以随机数字表法分为对照组(41例,生理盐水)和右美托咪定组(42例,右美托咪定)。比较两组的麻醉效果、血流动力学指标、氧化应激指标、炎性因子指标及心肌损伤标志物。结果右美托咪定组的麻醉效果满意度高于对照组(P<0.05)。T_(2)时,右美托咪定组的平均动脉压(MAP)、心率低于对照组(P<0.05)。术后1 d,右美托咪定组的超氧化物歧化酶(SOD)水平高于对照组,丙二醛(MDA)水平低于对照组(P<0.05)。术后3 d,两组的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)水平均较术前升高,但右美托咪定组低于对照组(P<0.05)。结论右美托咪定应用于非体外循环下心脏不停跳冠状动脉搭桥术中可减轻炎症反应、氧化应激损伤及心肌损伤,稳定血流动力学。
Objective To investigate the application effect of dexmedetomidine in off-pump coronary artery bypass grafting.Methods A total of 83 patients with coronary heart disease treated in our hospital from November 2020 to March 2021 were divided into control group(41 cases,normal saline)and dexmedetomidine group(42 cases,dexmedetomidine)by random number table method.The anesthetic effect,hemodynamic indexes,oxidative stress indexes,inflammatory factors indexes and myocardial injury markers were compared between the two groups.Results The anesthesia effect satisfaction in the dexmedetomidine group was higher than that in the control group(P<0.05).At T2,the mean arterial pressure(MAP)and heart rate in the dexmedetomidine group were lower than those in the control group(P<0.05).At 1 d after operation,the level of superoxide dismutase(SOD)in the dexmedetomidine group was higher than that in the control group,and the level of malondialdehyde(MDA)was lower than that in the control group(P<0.05).At 3 d after operation,the levels of interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),cardiac troponin I(cTnI)and creatine kinase isoenzyme(CK-MB)in the two groups were higher than those before operation,but those in the dexmedetomidine group were lower than the control group(P<0.05).Conclusion Dexmedetomidine applied in off-pump coronary artery bypass grafting can reduce inflammatory reaction,oxidative stress injury and myocardial injury,and stabilize hemodynamics.
