摘要
目的 探讨转化生长因子β1(TGF-β1)与解整合素-金属蛋白酶33(ADAM33)在小儿支气管哮喘外周血中的表达水平及其与肺功能的相关性。方法 选取2017年5月—2021年1月在本院呼吸内科确诊的小儿支气管哮喘患者共90例为病例组,其中轻度持续组40例、中度持续组50例;另取本院健康体检儿童88例为对照组。测定两组研究对象外周血TGF-β1、ADAM33水平、血清IgE、IL-4、IL-10水平和肺功能水平。结果 病例组外周血TGF-β1、ADAM33水平和血清IgE、IL-4、IL-10水平高于对照组,FEV1、FEV1/FVC、DLCO低于对照组(t=16.362、14.698、17.548、19.698、18.987、20.321、17.695、17.215,P均<0.001)。轻度持续组、中度持续组外周血TGF-β1、ADAM33水平和血清IgE、IL-4、IL-8、IL-10水平高于对照组,中度持续组外周血TGF-β1、ADAM33水平和血清IgE、IL-4、IL-8、IL-10水平高于轻度持续组。轻度持续组、中度持续组FEV1、FEV1/FVC、DLCO低于对照组,中度持续组FEV1、FEV1/FVC、DLCO低于轻度持续组(t=16.548、18.415、17.241、19.325、18.965、17.521、18.654、18.654,P均<0.001)。TGF-β1、ADAM33与FEV1、FEV1/FVC、DLCO负相关(r=-0.632、-0.636、-0.535、-0.718、-0.468、-0.509,P=0.019、0.013、0.020、0.017、0.032、0.012),与IgE、IL-4、IL-8正相关(r=0.494、0.522、0.786、0.763、0.445、0.414,P=0.031、0.030、0.014、0.014、0.042、0.025)。TGF-β1、ADAM33两者诊断小儿支气管哮喘的AUC、灵敏度、特异度相近(P>0.05),皆小于TGF-β1+ADAM33(P<0.05)。结论 支气管哮喘患儿外周血TGF-β1、ADAM3水平升高,且与疾病严重程度及肺功能相关;TGF-β1联合ADAM33诊断小儿支气管哮喘的灵敏度、特异度较高,可在临床上应用。
ObjectiveTo investigate the expressions of transforming growth factor β1(TGF-β1) and disintegrin-me-talloproteinase 33(ADAM33) in peripheral blood of children with bronchial asthma and their correlations with lung func-tion.MethodsA total of 90 cases of pediatric bronchial asthma diagnosed in the Department of Respiratory Medicine ofour hospital from May 2017 to January 2021 were selected as the case group, including 40 cases in the mild persistent sub-group and 50 cases in the moderate persistent sub-group. Eighty-eight healthy children for physical examinations in ourhospital were served as the control group. The levels of peripheral blood TGF-β1, ADAM33, serum IgE, IL-4, and IL-10,and the levels of lung function of the subjects of the two groups were measured.ResultsThe levels of peripheral bloodTGF-β1 and ADAM33, and serum IgE, IL-4, and IL-10 in the case group were higher than those in the control group, whilethe levels of FEV1, FEV1/FVC, and DLCO were lower than those in the control group(t = 16.362, 14.698, 17.548, 19.698,18.987, 20.321, 17.695, 17.215, all P < 0.001). The levels of peripheral blood TGF-β1 and ADAM33 and serum IgE, IL-4,IL-8, and IL-10 in the mild persistence sub-group and moderate persistence sub-group were higher than those of the controlgroup, and the levels of peripheral blood TGF-β1 and ADAM33, and serum IgE, IL-4, IL-8, IL-10 in the moderate persis-tence sub-group were higher than those in the mild persistent subgroup. The levels of FEV1, FEV1/FVC and DLCO in themild persistence sub-group and the moderate persistence sub-group were lower than those of the control group, and the levelsof FEV1, FEV1/FVC and DLCO of the moderate persistence sub-group were lower than those in the mild persistence subgroup(t =16.548, 18.415, 17.241, 19.325, 18.965, 17.521,18.654, 18.654, all P < 0.001). TGF-β1 and ADAM33were negatively correlated with FEV1, FEV1/FVC, and DL-CO(r =-0.632,-0.636,-0.535,-0.718,-0.468,-0.509,P = 0.019, 0.013, 0.020, 0.017, 0.032, 0.012), but positively correlated with
