可变翻转角的快速自旋回波双反转恢复序列在膝关节滑膜炎诊断中的价值 被引量:4

摘要 目的探讨可变翻转角的快速自旋回波双反转恢复(SPACE-DIR)序列在膝关节滑膜炎诊断中的价值。方法纳入49例疑似膝关节滑膜炎的患者,均予行常规序列、SPACE-DIR序列与对比增强脂肪抑制T_(1)WI(CE-FS-T_(1)WI)序列MRI扫描。由两名放射科医师评估每例患者髌上、内侧髌周、外侧髌周、髌下、髁间5个区域有无滑膜增厚。比较SPACE-DIR序列和CE-FS-T_(1)WI序列的图像整体质量评分与滑膜显示度评分;采用Kappa分析评价两种序列检查方法的诊断一致性,采用组内相关系数(ICC)分析观察者一致性。结果SPACE-DIR序列与CE-FS-T_(1)WI序列的图像整体质量评分、滑膜显示度评分比较,差异均无统计学意义(均P>0.05)。SPACE-DIR序列与CE-FS-T_(1)WI序列在髌上、内侧髌周、外侧髌周、髌下、髁间区域的诊断一致性达到较高或极高水平,Kappa系数分别为0.779、0.939、0.851、0.663、0.611(均P<0.05)。两名放射科医师应用SPACE-DIR序列在髌上、内侧髌周、外侧髌周区域的诊断一致性达到极高水平,ICC分别为0.85、0.82、0.93,而在髌下、髁间区域的诊断一致性较高,ICC分别为0.79、0.67(均P<0.05);两者应用CE-FS-T_(1)WI序列在上述5个区域的诊断一致性均达到极高水平,ICC分别为0.86、0.94、0.93、0.96、0.88(均P<0.05)。结论SPACE-DIR序列与CE-FS-T_(1)WI序列诊断膝关节滑膜炎的一致性较高,SPACE-DIR序列或可作为膝关节滑膜炎的MRI常规筛查序列。 Objective To investigate the value of sampling perfection with application optimized contrasts by using different flip angle evolution-double inversion recovery(SPACE-DIR)sequence in the diagnosis of knee joint synovitis.Methods A total of 49 patients with suspected knee joint synovitis were enrolled and underwent MRI scanning using conventional sequence,SPACE-DIR sequence,and contrast-enhanced fat-suppressed T_(1)WI(CE-FS-T_(1)WI)sequence.Two radiologists evaluated whether there was synovial thickening in the five areas of each patient,including suprapatellaris,medial patellaris,lateral patellaris,infrapatellaris and intercondylaris.The overall image quality score and synovial display score were compared between SPACE-DIR sequence and CE-FS-T_(1)WI sequence.Kappa analysis was used to evaluate the diagnostic consistency of the two methods of sequence examination,and the intraclass correlation coefficient(ICC)was employed to analyze the consistency between observers.Results There were no statistically significant differences in the overall image quality score or synovial display score between SPACE-DIR sequence and CE-FS-T_(1)WI sequence(all P>0.05).The diagnostic consistency of SPACE-DIR sequence and CE-FS-T_(1)WI sequence in suprapatellaris,medial patellaris,lateral patellaris,infrapatellaris,and intercondylaris areas reached a relatively high or very high level,and the Kappa coefficients were 0.779,0.939,0.851,0.663,and 0.611,respectively(all P<0.05).The consistency of two radiologists using SPACE-DIR sequence for diagnosis in the areas of suprapatellaris,medial patellaris,and lateral patellaris reached a very high level,exhibiting the ICCs of 0.85,0.82 and 0.93,respectively,whereas in the areas of infrapatellaris and intercondylaris reached a relatively high level,exhibiting the ICCs of 0.79 and 0.67,respectively(all P<0.05).The diagnostic consistency of the two using CE-FS-T_(1)WI sequence in the five areas mentioned above reached a very high level,and the ICCs were 0.86,0.94,0.93,0.96,and 0.88,respectively(a
