髌骨横行骨折应用关节镜下微创加压螺钉内固定治疗的临床研究 被引量:1

摘要 目的分析髌骨横行骨折患者采用关节镜下微创加压螺钉内固定术治疗的效果及对患者膝关节功能的影响。方法选取2018年6月至2020年1月期间在佳木斯市中心医院诊治的124例髌骨横行骨折患者,通过随机数字表分为对照组和观察组。对照组(62例)给予传统克氏针联合张力带内固定术治疗,观察组(62例)行关节镜下微创无头加压螺钉内固定术治疗。对比两组相关临床指标和膝关节活动度,参照Bostman评价表评估患者术后随访6个月的膝关节功能,分析并发症发生情况。结果相比对照组,观察组手术时间和骨折愈合时间缩短,术后骨折间距0 mm患者比例(87.10%)高,VAS评分降低;随访6个月,观察组患者伸直角度、屈曲角度和活动度均明显大于对照组,与健侧比较,活动度损失角度小于对照组;观察组患者膝关节功能优良率(96.78%)高于对照组,并发症总发生率(3.24%)低于对照组,两组比较差异有统计学意义(P<0.05)。结论髌骨横行骨折患者行关节镜下微创加压螺钉内固定术治疗时,手术时间缩短,骨折愈合加快,术后骨折间距小,膝关节功能恢复快,术后并发症减少。 Objective To analyze the effect of arthroscopic minimally invasive compression screw internal fixation on transverse patellar fracture and its effect on knee function.Methods 124 patients with transverse patellar fracture treated in The Central Hospital of Jiamusi City from June 2018 to January 2020 were randomly divided into control group and observation group.The control group(62 cases)were treated with traditional Kirschner wire combined with tension band internal fixation,and the observation group(62 cases)were treated with arthroscopic minimally invasive headless compression screw internal fixation.And then,the related clinical indexes and knee ranges of motion of the two groups were compared,the knee functions of the patients were evaluated according to Bostman evaluation table,and complications were analyzed.Results Compared with the control group,the operation time and fracture healing time of the observation group were shortened,the proportion of patients with 0 mm postoperative fracture spacing(87.10%)was high,and the VAS score was low.After 6 months of follow-up,the extension angle,flexion angle and range of motion of the patients in the observation group were significantly greater than those in the control group,and compared with the healthy side,the loss angle of range of motion was less than that in the control group.The excellent and good rate of knee function in the observation group(96.78%)was significantly higher than that in the control group,and the total incidence of complications(3.24%)was lower than that in the control group,and difference was statistically significant between the two groups(P<0.05).Conclusion Arthroscopic minimally invasive compression screw internal fixation for patients with transverse patellar fracture can shorten operation time,accelerate fracture healing,narrow postoperative fracture spacing,speed up the recovery of knee function,and reduce postoperative complications.
