摘要
目的探讨关节镜下胫骨四隧道技术治疗前交叉韧带胫骨止点撕脱骨折的临床疗效。方法回顾性分析2015年3月至2017年3月采用关节镜下胫骨四隧道技术治疗的25例前交叉韧带胫骨止点撕脱骨折患者临床资料,通过Lysholm评分及国际膝关节文献委员会(IKDC)评分评估术后膝关节功能。结果患者均获随访12~24个月,平均(18.5±2.1)个月。术前Lysholm评分为(42.9±4.9)分,提高至末次随访时的(92.3±2.7)分,术前IKDC评分为(36.4±4.0)分,提高至末次随访时的(90.0±2.1)分,比较差异均有统计学意义(P<0.001)。结论关节镜下胫骨四隧道技术治疗成人前交叉韧带胫骨止点撕脱骨折可达到创伤小、固定牢固、功能恢复良好的临床疗效。
Objective To investigate the clinical effect of arthroscopic four-tunnel technique in the treatment of the tibial avulsion fractures of anterior cruciate ligament.Methods The clinical data of 25 patients with tibial avulsion fracture of anterior cruciate ligament treated with arthroscopic four-tunnel technique from March 2015 to March 2017 were retrospectively analyzed,and the postoperative knee function was evaluated with Lysholm score and International Knee Documentation Committee(IKDC)score.Results All patients were followed up for 12 to 24 months after discharge,with an average of(18.5±2.1)months.The Lysholm score was increased from preoperative(42.9±4.9)points to(92.3±2.7)points at the last follow-up visit,and the preoperative IKDC score from preoperative(36.4±4.0)points to(90.0±2.1)points at the last visit.The differences were statistically significant(P<0.001).Conclusion Arthroscopic tibial four-tunnel technique has the clinical efficacies of less trauma,firm fixation and good functional recovery in the treatment of adult tibial avulsion fractures of anterior cruciate ligament.
