摘要
目的 探讨同型半胱氨酸(Hcy)、丙二醛(MDA)、超氧化物歧化酶(SOD)与血管性痴呆(VD)病情程度和进展速度的关系。方法 回顾性分析2018年2月-2021年6月收治的83例VD患者的临床资料,选择同期体检的50例健康者作为对照组,比较2组血清学指标;根据VD患者1年后MMSE评分变化值将患者分为快速进展组37例和缓慢进展组46例,比较2组患者临床资料和血清学指标;应用Logistic回归分析VD疾病进展速度的影响因素;采用Pearson相关性分析血清学指标与MMSE评分变化值的关系。结果 VD组Hcy、MDA水平显著高于对照组(P <0.05),SOD水平显著低于对照组(P <0.05);随着病情的加重,VD患者Hcy、MDA水平呈上升趋势(P <0.05),SOD水平呈下降趋势(P <0.05);快速进展组患者病程、空腹血糖、TG、Hcy、MDA水平显著高于缓慢进展组(P <0.05),HDL-C、SOD水平显著低于缓慢进展组(P <0.05)。Logistic回归分析显示,病程、TG、Hcy、MDA、SOD是影响疾病进展速度的主要因素(P <0.05)。Pearson相关性分析显示,VD患者血清Hcy、MDA水平与MMSE评分变化值成正相关(r值分别为0.740、0.696,P <0.05),SOD水平与MMSE评分成负相关(r=-0.750,P <0.05)。结论 VD患者Hcy、MDA、SOD与病情程度和疾病进展速度关系密切,可为临床评估VD病情程度和预后提供参考依据。
Objective To investigate the relationship between homocysteine( Hcy),malondialdehyde( MDA),superoxide dismutase( SOD) and the severity and progression rate of vascular dementia( VD). Methods The clinical data of 83 VD patients admitted from February 2018 to June 2021 were retrospectively analyzed. 50 healthy subjects at the same period were selected as the control group,and the serological indexes of the two groups were compared. According to the MMSE score after1 year,patients were divided into the rapid progression group( n = 37) and the slow progression group( n = 46),and the clinical data and serological indicators of the two groups were compared. Logistic regression was used to analyze the influencing factors of VD disease progression rate,and Pearson correlation was used to analyze the relationship between serological indexes and MMSE score. Results Hcy and MDA levels in VD group were significantly higher than those in control group( P < 0. 05),while the SOD level was significantly lower than that in control group( P < 0. 05). As the disease gets worse,the levels of Hcy and MDA were increased( P < 0. 05),while the levels of SOD were decreased( P < 0. 05). The course of disease,fasting blood glucose,TG,Hcy and MDA levels in fast progression group were significantly higher than those in slow progression group( P < 0. 05),while the levels of HDL-C and SOD in fast progression group were significantly lower than those in slow progression group( P < 0. 05). Logistic regression analysis showed that the course of disease,TG,Hcy,MDA and SOD were the main factors affecting the rate of disease progression( P < 0. 05). Pearson correlation analysis showed that serum Hcy and MDA levels were positively correlated with MMSE score in VD patients( r = 0. 740,r = 0. 696,P < 0. 05),while SOD level was negatively correlated with MMSE score( r =-0. 750,P < 0. 05). Conclusion Hcy,MDA and SOD are closely related to the severity and progression of VD,which can provide reference for clinical evaluation of the severity and prognosis.
