摘要
目的探讨尖锐湿疣(CA)患者复发的影响因素及与皮损局部辅助性T细胞1/辅助性T细胞2(Th1/Th2)免疫失衡的相关性。方法选取2019年1月至2020年6月第四军医大学西京皮肤医院收治的224例CA患者作为研究对象。随访1年,统计复发率,并根据复发情况分为复发组和未复发组。比较两组的基线资料、Th1细胞因子[白介素-2(IL-2)、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)]和Th2细胞因子[白介素-4(IL-4)、白介素-6(IL-6)、白介素-10(IL-10)]水平。构建预测复发的Logistic回归模型,分析模型及Th1/Th2细胞因子的预测价值。结果文化程度、IL-2、IFN-γ、TNF-α是复发的保护因素,饮酒、性伴侣数量、性伴侣伴有CA、经常熬夜、IL-4、IL-6、IL-10是复发的危险因素(P<0.05);Logistic回归模型预测复发的曲线下面积(AUC)为0.742;IL-2、IFN-γ、TNF-α、IL-4、IL-6、IL-10联合预测复发的AUC为0.890。结论文化程度、饮酒、性伴侣数量、性伴侣伴有CA、经常熬夜及皮损局部Th1/Th2免疫失衡均与CA复发有关,联合检测皮损局部Th1/Th2细胞因子有望成为量化预测CA复发的手段之一。
Objective To investigate the factors affecting the recurrence of condyloma acuminatum(CA)and its correlation with the local T helper cell 1/T helper cell 2(Th1/Th2)immune imbalance in skin lesions.Methods 224 patients with CA admitted to Xijing Dermatology Hospital,Fourth Military Medical University from January 2019 to June 2020 were selected as the study objects.The recurrence rate was counted at 1 year of follow-up,and divided into recurrence group and non-recurrence group according to the recurrence status.Baseline data,Th1 cytokine[interleukin-2(IL-2),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α)]and Th2 cytokine[interleukin4(IL4),interleukin-6(IL-6),interleukin-10(IL-10)]levels were compared between the two groups.Logistic regression models for predicting relapse were constructed,and the predictive value of the models and Th1/Th2 cytokines was analyzed.Results Degree of education,IL-2,IFN-γ and TNF-α were protective factors for recurrence;drinking,number of sexual partners,sexual partners with CA,frequent stay up late,IL4,IL6 and IL-10 were risk factors for recurrence(P<0.05).The area under curve(AUC)of recurrence predicted by Logistic regression model was 0.742.The AUC of IL-2、IFN-γ、TNF-α、IL-4、IL-6 and IL-10 combined prediction of recurrence was 0.890.Conclusions Degree of education,drinking,number of sexual partners,sexual partners with CA,frequent staying up late,and local Th1/Th2 immune imbalance in skin lesions are all related to CA recurrence.Combined detection of local Th1/Th2 cytokines in skin lesions is expected to be one of the means to quantitatively predict CA recurrence.
