摘要
目的探讨隐匿精子症患者不同周期使用射出精子和睾丸精子进行卵胞浆内单精子注射(ICSI)后的妊娠结局。方法选取2010年1月至2014年12月成都市妇女儿童中心医院收治的21对(共111个周期)进行ICSI助孕治疗的隐匿精子症患者夫妇作为研究对象,进行回顾性分析。根据精子来源分为射出精子组(n=65)和睾丸精子组(n=46)。比较两组的基线资料、受精及胚胎发育情况。结果睾丸精子组的女方年龄、男方年龄均显著大于射出精子组,差异具有统计学意义(P<0.05)。两组活动精子百分率、双原核(2PN)率、2PN卵裂率、优质胚胎率比较,差异无统计学意义(P>0.05);睾丸精子组种植率、妊娠率显著高于射出精子组,差异具有统计学意义(P<0.05)。结论隐匿精子症患者采用睾丸精子会获得更高的种植率和妊娠率。
Objective To discuss the pregnancy outcomes of patients with cryptozoospermia after intracytoplasmic sperm injection(ICSI) with ejaculated sperm and testicular sperm in different cycles. Methods 21 couples with cryptozoospermia who received ICSI assisted pregnancy treatment(111 cycles in total) in Chengdu Women′s and Children′s Central Hospital from January 2010 to December 2014 were selected as the research objects for retrospective analysis. According to the origin of sperm, they were divided into ejaculatory sperm group(n=65) and testicular sperm group(n=46). Baseline data, fertilization and embryo development were compared between the two groups. Results The age of female and male in testicular sperm group was significantly higher than that in ejaculated sperm group(P<0.05). There were no significant differences in the percentage of motile sperm, di-pronucleus(2 PN) rate, 2 PN cleavage rate and high-quality embryo rate between the two groups(P>0.05). The implantation rate and pregnancy rate of testicular sperm group were significantly higher than those of ejaculated sperm group(P<0.05). Conclusions Testicular sperm is associated with higher implantation rates and pregnancy rates in patients with cryptozoospermia.
