摘要
目的检索、评价并总结维持性血液透析患者抗凝管理的最佳证据,为完善维持性血液透析患者抗凝管理提供参考。方法系统检索美国指南网、英国国家卫生与临床优化研究所指南网、安大略注册护士协会网站、国际指南协作网、新西兰准则小组、澳大利亚国家健康和医学研究委员会、苏格兰学院间指南网、中国医脉通、美国肾脏病基金会、改善全球肾脏病预后组织、欧洲肾脏最佳临床实践、英国肾脏病学会、中华医学会肾脏病学分会、国家糖尿病消化与肾脏病研究所、BMJ Best Practice、UpToDate、Joanna Briggs Institute循证卫生保健中心数据库、Cochrane Library、Nursing consult、Web of Science、PubMed、Embase、Scopus、中国知网、万方数据库、维普数据库和中国生物医学文献数据库中有关维持性血液透析抗凝管理的证据,内容包括指南、临床决策、证据总结、最佳临床实践、推荐实践、专家共识及系统评价。结果共纳入文献17篇,包括10篇证据总结,3篇指南,1篇临床决策,3篇推荐实践,包括人员配置、抗凝指征评估、患者健康教育、护理人员培训、无出血风险患者抗凝,高出血风险患者抗凝等方面汇总23条证据。结论医护人员可结合所在机构现状,选择性应用证据,为患者制定个性化抗凝管理方案,从而提高护理质量。
Objective To retrieve,assess,and summarize the best evidence of anticoagulation management in maintenance hemodialysis patients and to provide the reference for improving anticoagulant management in them.Methods Systematic retrieves for evidence of anticoagulation management in maintenance hemodialysis patients were performed by National Comprehensive Cancer Network,Britain's National Institute for Health and Care Excellence Guide Network,Ontario Registered Nurses Association Website,International Guidelines for the Network,New Zealand Criteria Group,Australia's National Institute of Health and Medical Research Council,Scottish Intercollegiate Guidelines Network,Chinese Medical pulse flux,National Kidney Foundation,Kidney Disease:Improving Global Outcomes,European Renal Best Practice,The Renal Association Chinese Society of Nephrology,National Institute of Diabetes Digestive and Renal Disease,BMJ Best Practice,UpToDate,Joanna Briggs Institute Database of Evidence-based Health Care Centers,Cochrane Library,Nursing consult,Web of Science,PubMed,Embase,Scopus,CNQI,Wanfang database,VIP database,and Chinese Biomedical Literature database,including guidelines,clinical decision,evidence summary,the best practice information sheet,recommended practice,expert consensus,and systematic review.Results A total of 17 articles were included,including 10 evidence summary,3 guidelines,1 clinical decision,and 3 recommended practices.Six aspects with 23 pieces of evidence were summarized in terms of staffing,evaluation of anticoagulation indications,patient health education,nursing staff training,anticoagulation in patients without bleeding risk,and patients with high bleeding risk.Conclusion Based on the current situation of the institution and the selective application of evidence,medical staff can develop personalized anticoagulant management plans for patients,thus improving the quality of care.
