摘要
目的探讨针灸结合切开减压术治疗小腿骨筋膜室综合征的临床效果。方法纳入2018年6月至2020年8月在西南医科大学附属中医医院骨科住院治疗小腿骨筋膜室综合征病人68例。采用随机数字表法将病人分为对照组和研究组,对照组病人(n=34)单纯采取切开减压术,研究组病人(n=34)在切开减压术的基础上加用针灸治疗,比较两组总体临床疗效和满意度、治疗前后不同时间段疼痛视觉模拟评分法(visual analoguescale,VAS)评分及Fugl-Meyer运动功能评分法(FMA)评分。结果研究组临床疗效总有效率(94.12%)高于对照组(76.47%),差异有统计学意义(χ^(2)=4.221,P=0.04);研究组满意度(94.12%)高于对照组(73.53%),差异具有统计学意义(χ^(2)=5.314,P=0.021);治疗后1周和治疗后1月研究组病人的VAS评分明显低于对照组,差异均具有统计学意义(P<0.05);治疗后1周和治疗后1月研究组病人的FMA评分明显高于对照组,差异均具有统计学意义(P<0.05)。结论针灸配合切开减压治疗小腿骨筋膜室综合征有较好的临床疗效,可有效地缓解病人术后疼痛,且能促进病人术后肢体功能障碍恢复,可在临床推广应用。
Objective To explore the clinical effect of acupuncture combined with incision decompression in the treatment of compartment syndrome in calf muscle.Methods A total of 68 patients diagnosed with calf compartment syndrome during hospitaliza⁃tion in the Department of Orthopedics,the Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University from June 2018 to August 2020 were included.The patients were divided into control group and study group by random number table method.Pa⁃tients in the control group(n=34)underwent incision decompression alone,while patients in the study group(n=34)were treated with acupuncture in addition to incision decompression.The overall clinical efficacy and satisfaction,pain visual analogue scale(VAS)score and Fugl-Meyer motor function score(FMA)score at different time periods before and after treatment were compared between the two groups.Results The total effective rate of clinical efficacy in the study group(94.12%)was higher than that of the control group(76.47%),and the difference was statistically significant(χ^(2)=4.221,P=0.04);the satisfaction in the study group(94.12%)was higher than that of the control group(73.53%),and the difference was statistically significant(χ^(2)=5.314,P=0.021);VAS score of 1 week and 1 month after treatment in the study group was significantly lower than that in the control group,and the differences were statisti⁃cally significant(P<0.05);The FMA score in the study group was significantly higher than that of the control group after treatment du⁃ration of one week and one month respectively(P<0.05).Conclusion Acupuncture combined with incision decompression had great clinical efficacy in the treatment of calf compartment syndrome,which can effectually relieve postoperative pain and promote the recov⁃ery of postoperative limb dysfunction.It is worthy of further clinical promotion.
