摘要
目的探讨经阴道三维超声成像技术在宫腔病变及子宫畸形诊断中的应用效果。方法将2020年6月至2022年1月在开封市妇产医院接受超声诊断检查的妇科患者620例作为本次观察对象,入院后均行经阴道二维超声检查,随后更改频数为三维超声,以宫腔镜和病理检查结果为金标准,比较不同检查方式诊断宫腔病变及子宫畸形的符合率。结果经宫腔镜和病理检查宫腔病变及子宫畸形者共60例,占比9.68%。其中宫腔病变率85%,子宫畸形发生率15%;经阴道二维超声检查宫腔病变及子宫畸形者共43例,其中宫腔病变符合率72.55%,子宫畸形符合率66.67%;经阴道三维超声检查宫腔病变及子宫畸形者共55例,其中宫腔病变符合率92.16%,子宫畸形符合率88.89%,三种检出结果比较,差异有统计学意义(χ^(2)=5.339,P<0.05);经阴道三维超声诊断子宫内膜息肉、子宫黏膜下肌瘤、宫腔粘连、纵膈子宫的符合率为92.0%、92.31%、87.5%、100.00%高于经阴道二维超声诊断符合率80.00%、69.23%、62.50%、60.00%,差异有统计学意义(P<0.05);经阴道三维超声的诊断符合率高于二维超声;且经阴道三维超声在诊断准确度、敏感度和特异度方面均高出经阴道二维超声,差异有统计学意义(P<0.05)。结论经阴道三维超声成像技术对宫腔病变和子宫畸形的诊断准确度、敏感度和特异度较高,可获得更多更有效的子宫异常信息,从而为临床制定后续治疗方案提供出准确依据。
Objective To investigate the effect of transvaginal three-dimensional ultrasound imaging in the diagnosis of uterine cavity lesions and uterine malformations.Methods A total of 620 gynecological patients who underwent ultrasound diagnosis in Kaifeng maternity hospital from June 2020 to January 2022 were observed.After admission,all patients underwent transvaginal two-dimensional ultrasound,and then changed the frequency to three-dimensional ultrasound.Taking the results of hysteroscopy and pathological examination as the gold standard,the coincidence rates of different examination methods in diagnosing uterine lesions and uterine malformations were compared.Results A total of 60 cases(9.68%)of uterine cavity lesions and uterine malformations were examined by hysteroscopy and pathology,the incidence of uterine cavity lesions was 85%,and the incidence of uterine malformations was 15%;There were 43 cases of uterine cavity lesions and uterine malformations examined by transvaginal two-dimensional ultrasound,of which the coincidence rate of uterine cavity lesions was 72.55%,and the coincidence rate of uterine malformations was 66.67%;A total of 55 cases of intrauterine lesions and uterine malformations were examined by transvaginal three-dimensional ultrasound,of which the coincidence rate of intrauterine lesions was 92.16%,and the coincidence rate of uterine malformations was 88.89%,the difference between the three detection results was statistically significant(χ_(2)=5.339,P<0.05);The coincidence rates of transvaginal three-dimensional ultrasound in the diagnosis of endometrial polyps,submucous myomas,intrauterine adhesions,and mediastinal uterus were 92.0%,92.31%,87.5%,100.00%,which were higher than those of transvaginal two-dimensional ultrasound(80.00%,69.23%,62.50%,60.00%),the difference was statistically significant(P<0.05);The diagnostic accuracy of transvaginal three-dimensional ultrasound was higher than that of two-dimensional ultrasound;The diagnostic accuracy,sensitivity and specificity of transvaginal t
