摘要
目的探讨毫刃针配合美式整脊治疗神经根型颈椎病临床效果。方法按随机数字表法将2019年5月至2021年5月期间河南推拿职业学院附属医院骨关节病科收治的神经根型颈椎病患者(N=86)分为研究组(N=43,毫刃针配合美式整脊治疗)及对照组(N=43,传统针灸治疗)。观察两组患者治疗效果,并采用专用量表调查两组患者治疗前后疼痛程度、颈椎功能、颈椎症状;比较两组治疗前后超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)和降钙素相关肽(CGRP)水平。结果研究组的治疗有效率(95.35%)高于对照组(79.07%),差异有统计学意义(P<0.05);治疗前,两组视觉模拟评分表(VAS)、颈椎功能障碍指数(NDI)得分差异无统计学意义(P>0.05);治疗28 d后,研究组的VAS及NDI得分均低于对照组,差异有统计学意义(P<0.05);治疗前,两组颈肩部、手指及上肢症状得分差异无统计学意义(P>0.05);治疗28 d后,研究组颈肩部、手指及上肢症状得分均高于对照组,差异有统计学意义(P<0.05)。两组治疗前炎症指标比较,差异无统计学意义(P>0.05);治疗后,两组hs-CRP、TNF-α、IL-1β和CGRP水平均低于本组治疗前,差异有统计学意义(P<0.05),且研究组治疗后上述指标水平低于对照组,差异有统计学意义(P<0.05)。结论毫刃针配合美式整脊治疗神经根型颈椎病疗效确切、操作便捷,可明显改善患者的临床症状,其机制可能与下调血清hs-CRP、TNF-α、IL-1β和CGRP等指标有关。
Objective To discuss the clinical effects of blade needle and American chiropractic in treating the cervicalspondylotic radiculopathy.Methods A total of 86 patients with cervicalspondylotic radiculopathy treated in our osteoarthropathy department from May 2019 to May 2021 were selected and randomly assigned to study group(N=43,blade needle combined with American chiropractic)and control group(N=43,traditional acupuncture therapy)by the random number table method.Treatment outcomes were evaluated through the effective rate,pain degree,cervical vertebral function,cervical vertebral symptoms,hypersensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and calcitonin gene-related peptide(CGRP).Results The effective rate in the study group(95.35%)was higher than control group(79.07%)(P<0.05);Before treatment,VAS(visual analogue scale)and NDI(neck disability index)scores between two groups were not significantly different(P>0.05);After 28 d of treatment,VAS and NDI scores in the study group were lower than those in the control group(P<0.05);Before treatment,the neck,shoulder,finger and upper limb symptom scores between two groups were not significantly different(P>0.05);After 28 d of treatment,the neck,shoulder,finger and upper limb symptom scores in the study group were higher than those in the control group(P<0.05);Before treatment,inflammatory factors between two groups were not significantly different(P>0.05);After treatment,hs-CRP,TNF-α,IL-1βand CGRP levels in two groups were lower than before(P<0.05);Related indicators in the study group were even lower than those in the control group(P<0.05).Conclusion The blade needle combined with American chiropractic treatment of cervicalspondylotic radiculopathy is effective and convenient to operate,it can effectively reduce the clinical symptoms,and its mechanism may be correlated with the down-regulated hs-CRP,TNF-α,IL-1βand CGRP levels.
