摘要
目的研究低钙腹膜透析液对慢性肾衰竭患者血清钙磷代谢、血红蛋白及残余肾功能的影响。方法按随机数字表法将2019年6月至2021年6月入住平顶山市第一人民医院的118例慢性肾衰竭患者分为观察组(N=59)和对照组(N=59)。观察组采用低钙腹膜透析液透析治疗;对照组采用标准钙腹膜透析液透析治疗。观察两组患者血清钙磷代谢水平、血红蛋白水平、全段甲状旁腺激素水平、肾功能和残余肾功能水平以及肾损伤标志物水平。随访患者6个月,观察两组患者并发症情况。结果观察组钙磷离子水平均低于对照组,血红蛋白水平高于对照组,差异有统计学意义(P<0.05);两组血清全段甲状旁腺激素(iPTH)水平比较,差异无统计学意义(P>0.05)。观察组血尿素氮(BUN)、血肌酐(Scr)水平均低于对照组,残余肾功能(RRF)水平高于对照组,差异有统计学意义(P<0.05)。观察组白介素-18(IL-18)和尿肾损伤分子(Kim-1)水平均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为6.78%,对照组为10.17%,两组患者并发症发生率对比,差异无统计学意义(P>0.05)。结论低钙腹膜透析液有利于调节透析患者的钙磷水平,减轻患者的肾功能损伤,延缓残余肾功能水平的降低,值得临床应用。
Objective To study the effect of low calcium peritoneal dialysate on serum calcium and phosphorus metabolism,hemoglobin and residual renal function in patients with chronic renal failure.Methods A total of 118 patients with chronic renal failure admitted to Pingdingshan First People's Hospital from June 2019 to June 2021 were randomly divided into observation group(N=59)and control group(N=59)according to the random number table.The observation group was treated with low calcium peritoneal dialysate,the control group was treated with standard calcium peritoneal dialysis fluid.The levels of serum calcium and phosphorus metabolism,hemoglobin,whole segment parathyroid hormone,renal function,residual renal function and markers of renal injury were observed in the two groups.The patients were followed up for 6 months to observe the complications of the two groups.Results The levels of calcium and phosphorus ions in the observation group were lower than those in the control group,and the level of hemoglobin was higher than that in the control group,the difference was statistically significant(P<0.05);There was no significant difference in the level of serum parathyroid hormone(iPTH)between the two groups(P>0.05).The levels of blood urea nitrogen(BUN)and serum creatinine(Scr)in the observation group were lower than those in the control group,and the level of residual renal function(RRF)was higher than that in the control group,the difference was statistically significant(P<0.05).The levels of interleukin-18(IL-18)and urinary and renal injury molecule(KIM-1)in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of complications was 6.78%in the observation group and 10.17%in the control group,there was no significant difference between the two groups(P>0.05).Conclusion Low calcium peritoneal dialysis fluid is beneficial to regulate the calcium and phosphorus level of dialysis patients,reduce the renal function injury of patients,and delay
